Résumé : Opioid analgesia is counteracted on chronic use by tolerance and hyperalgesia inducing dose escalation and life-threatening overdoses. Mu opiate receptors (MOR) expressed in primary sensory neurons were recently found to control tolerance and hyperalgesia, but the underlying mechanisms remained elusive. Here we show that genetic inactivation of fms -like tyrosine kinase receptor 3 (FLT3) receptor in sensory neurons abrogates morphine tolerance and hyperalgesia by preventing MOR-induced hyperactivation of the cAMP signaling pathway and subsequent excitatory adaptive processes. Moreover, the specific FLT3 inhibitor BDT001 potentiates morphine analgesia in acute and chronic pain models, without aggravating morphine adverse effects, and reverses tolerance and hyperalgesia once installed. Thus, FLT3 appears as a key regulator of the MOR signaling pathway and its pharmacological blockade shows promise to enhance chronic opioid analgesic efficacy.

Résumé : Immunotherapy of Alzheimer's disease (AD) is a promising approach to reduce the accumulation of amyloid-beta (Aβ), a critical event in the onset of the disease. Targeting the group II metabotropic glutamate receptors, mGlu2 and mGlu3, could be important in controlling Aβ production, although their respective contribution remains unclear due to the lack of selective tools. Here, we show that enhancing mGlu2 receptor activity increases Aβ1-42 peptide production whereas activation of mGlu3 has no effect. We show that such a difference likely results from the direct interaction of APP with mGlu3, but not with mGlu2 receptors, that prevents APP amyloidogenic cleavage and Aβ1-42 peptides production. We then show that chronic treatments of the AD model 5xFAD mice with a brain-penetrating mGlu2-potentiating nanobody accelerated amyloid aggregation and exacerbated memory deficits, but had no effect in control mice. Our results confirm that a selective mGluR2 activation exacerbates AD disease development, suggesting that therapeutic benefices could be obtained with blockers of this receptor. Our study also provides the proof-of-concept that chronic administration of nanobodies targeting neuroreceptors can be envisioned to treat brain diseases.

Résumé : Abstract The interaction of several partners with Ku through Ku-binding motifs (KBMs) in their sequences governs their enrolment in NHEJ repair complexes. Here, we first established more specifically the function of KBMs in V(D)J recombination as the molecular basis of functional redundancy between XLF and the NHEJ proteins MRI and PAXX. Then, given the functional redundancy between RAG2 and XLF, we explored the hypothesis of a KBM-mediated interaction between RAG2 and Ku. Through sequence alignment and biophysical methods, we identified a KBM at the C-terminus of RAG2 (R2CT) that mediates its interaction with Ku both in vitro and in cellulo . Notably, we showed that R2CT/Ku interaction is independent of the RAG nuclease activity. Finally, we demonstrated that the respective KBMs of RAG2 and XLF support their functional redundancy for V(D)J recombination.

Résumé : Background: Head injuries are the leading cause of trauma in Emergency Departments (EDs). Recent studies have shown epidemiological changes in patients consulting ED for head injuries. The main objective of this study was to describe the profile of head injury patients consulting in the EDs in France and assess head injury severity across age groups; the delay between the occurrence of head injury and ED arrival; factors associated with traumatic intracranial hemorrhage (ICH). Methods: This cross-sectional study collected patient data over a three-day period in March 2023. All adult patients (≥ 18 years old) admitted to the ED with a head injury (defined as a trauma to the head) were included. TBI severity was classified according to patients' initial Glasgow Coma Scale score in the ED: severe (3-8); moderate (9 -12); mild (13-15); and simple head trauma in the absence of transient or persistent neurological symptoms. Results: Among the 71 participating EDs, 26,008 patients visited EDs and a total of 1070 patients (4.1%, IC 95 3.9-4.4) presented a head injury were included in the study, with a median age of 68.5 [37-85] years old. Most of the patients (66.7%) were referred to ED after a call to the Emergency Medical Dispatcher (EMD). The median time from head injury to ED visit was 2 h [1.0 - 5.5]. Ground-level falls were the leading cause of head injury (60.3%). Most of patient presented a simple head trauma (n = 715, 66.8%) followed by mild TBI (n = 337, 31.5%). CT head scans were performed for 636 patients (59.6%), of which 58 were positive. Traumatic ICH prevalence was 5.4% (95% CI: 4.1-6.9) and three patients (0.3%) required an urgent neurosurgical intervention. Neither preinjury anticoagulant (p = 0.97) nor antiplatelet (p = 0.93) use was associated with an increased risk of traumatic ICH. Conclusions: One head injury patient out of two presenting in the ED is aged over 65 years. Patients referred by EMD were more likely to visit ED promptly. The majority of older patients underwent a head CT scan and preinjury anticoagulant use was not associated with increased risk of traumatic ICH.

Résumé : Practical MRI training is essential for bridging the gap between complex theoretical knowledge and clinical applications. Traditional phantoms used in MRI, such as ACR phantoms, are valuable for illustrating system characterization methods but often lack the anatomical complexity required for realistic training. This study presents the development of high-fidelity anatomical phantoms designed specifically for practical MRI training. These phantoms replicate key anatomical structures and tissue contrasts of the head, offering a more realistic training experience for healthcare students and medical imaging staff (radiologists, physicists, radiographers). Materials and Methods: We focused on the head region, creating phantoms from reference MRI T1-weighted slices and using computer-aided design (CAD) software to design detailed anatomical structures. These phantoms were 3D printed and filled with tissue-mimicking gels. MRI acquisitions were performed using a 1.5T clinical MRI system.
Results: The resulting images demonstrated high anatomical fidelity and realistic MRI tissue contrasts. The phantoms allow for effective demonstration of the impact of parameter modifications on MRI images and aid in anatomical structure recognition. Conclusion: While technical improvements are needed to ensure long-term stability and accurate relaxometric properties, these phantoms hold significant potential for enhancing MRI education and sequences evaluations. The approach can be extended to other anatomical regions, further supporting the training and optimization of MRI sequences.

Résumé : In this Letter, the first evidence of the ${}^4_{\bar{\Lambda}}\overline{\mathrm{He}}$ antihypernucleus is presented, along with the first measurement at the LHC of the production of (anti)hypernuclei with mass number $A=4$, specifically (anti)${}^4_{\Lambda}\mathrm{H}$ and (anti)${}^4_{\Lambda}\mathrm{He}$. In addition, the antiparticle-to-particle ratios for both hypernuclei (${}^4_{\bar{\Lambda}}\overline{\mathrm{H}}$ / ${}^4_{\Lambda}\mathrm{H}$~and ${}^4_{\bar{\Lambda}}\overline{\mathrm{He}}$ / ${}^4_{\Lambda}\mathrm{He}$) are shown, which are sensitive to the baryochemical potential of the strongly-interacting matter created in heavy-ion collisions. The results are obtained from a data sample of central Pb--Pb collisions, collected during the 2018 LHC data-taking at a center-of-mass energy per nucleon pair of $\sqrt{s_{\mathrm{NN}}} = $ 5.02 TeV. The yields measured for the average of the charge-conjugated states are found to be $[0.78 \; \pm \; 0.19 \; \mathrm{(stat.)} \; \pm \; 0.17 \; \mathrm{(syst.)}] \times 10^{-6}$ for the (anti)${}^4_{\Lambda}\mathrm{H}$ and $[1.08 \; \pm \; 0.34 \; \mathrm{(stat.)} \; \pm \; 0.20 \; \mathrm{(syst.)}] \times 10^{-6}$ for the (anti)${}^4_{\Lambda}\mathrm{He}$, and the measured antiparticle-to-particle ratios are in agreement with unity. The presence of (anti)${}^4_{\Lambda}\mathrm{H}$ and (anti)${}^4_{\Lambda}\mathrm{He}$ excited states is expected to strongly enhance the production yield of these hypernuclei. The yield values exhibit a combined deviation of 3.3$\sigma$ from the theoretical ground-state-only expectation, while the inclusion of the excited states in the calculations leads to an agreement within 0.6$\sigma$ with the present measurements. Additionally, the measured (anti)${}^4_{\Lambda}\mathrm{H}$ and (anti)${}^4_{\Lambda}\mathrm{He}$ masses are compatible with the world-average values within the uncertainties.

Résumé : BACKGROUND: Isolated posterior leaflet mitral valve prolapse (PostMVP), a common form of MVP, often referred as fibroelastic deficiency, is considered a degenerative disease. PostMVP patients are usually asymptomatic and often undiagnosed until chordal rupture. The present study aims to characterize familial PostMVP phenotype and familial recurrence, its genetic background, and the pathophysiological processes involved.METHODS: We prospectively enrolled 284 unrelated MVP probands, of whom 178 (63%) had bi-leaflet MVP and 106 had PostMVP (37%). Familial screening within PostMVP patients allowed the identification of 20 families with inherited forms of PostMVP for whom whole genome sequencing was carried out in probands. Functional in vivo and in vitro investigations were performed in zebrafishand in Hek293T cells.RESULTS: In the 20 families with inherited form of PostMVP, 38.8% of relatives had a MVP/prodromal form, mainly of the posterior leaflet, with transmission consistent with an autosomal dominant mode of inheritance. Compared with control relatives, PostMVP family patients have clear posterior leaflet dystrophy on echocardiography. Patients with PostMVP present a burden of rare genetic variants in ARHGAP24. ARHGAP24 encodes the filamin A binding RhoGTPase-activating protein FilGAP and its silencing in zebrafish leads to atrioventricular regurgitation. In vitro functional studies showed that variants of FilGAP, found in PostMVP families, are loss-of-function variants impairing cellular adhesion and mechano-transduction capacities. CONCLUSIONS PostMVP should not only be considered an isolated degenerative pathology but as a specific heritable phenotypic trait with genetic and functional pathophysiological origins. The identification of loss-of-function variants in ARHGAP24 further reinforces the pivotal role of mechano-transduction pathways in the pathogenesis of MVP.CLINICAL PERSPECTIVE Isolated posterior mitral valve prolapse (PostMVP), often called fibro-elastic deficiency MVP, is at least in some patients, a specific inherited phenotypic trait PostMVP has both genetic and functional pathophysiological origins Genetic variants in the ARHGAP24 gene, which encodes for the FilGAP protein, cause progressive Post MVP in familial cases, and impair cell adhesion and mechano-transduction capacities

Résumé : Background Nodes are the second site for prostate cancer recurrence. Whole-pelvic radiotherapy (WPRT) has shown superiority over nodal stereotactic body radiotherapy (SBRT) in two retrospective cohorts. We aimed to compare both modalities and assess factors associated with treatment outcomes. Materials and methods This retrospective multicentric cohort study included patients from five institutions spanning from 2010 to 2022. Patients had a history of prostatic adenocarcinoma classified as N0 M0 at diagnosis with a first nodal-only pelvic castration-sensitive recurrence. Failure-free survival (FFS) was defined as the time from the end of RT to the first failure event–biochemical or imaging recurrence, or death. Results A total of 147 patients (pts) were analyzed, mainly treated for a recurrence after initial prostatectomy (87%), with 64 (43.5%) undergoing SBRT and 83 (56.5%) undergoing WPRT. SBRT was chosen mainly for dosimetric constraints (67%) and was associated with a lower rate of concomitant androgen deprivation therapy (ADT) prescription. With a median follow-up of 68 months [inter-quartile range (IQR) = 51], FFS was significantly lower in the SBRT group (p < 0.0001). In multivariable analysis, WPRT and ADT were associated with a longer FFS. Factors associated with a longer FFS after SBRT included associated ADT, lower prostate-specific antigen (PSA) levels, a PSA doubling time >6 months, and a Gleason score <8. SBRT was associated with a lower rate of genitourinary and gastrointestinal grade ≥2 complications. Discussion For an isolated pelvic nodal prostate cancer recurrence, SBRT is associated with a shorter FFS compared to WPRT. SBRT is often more convenient for patients and leaves further pelvic salvage options available, so it can be explored as an option for well-informed patients.

Résumé : For the wine industry, ensuring quality and authenticity hinges on the precise determination of wine origin. In our study, we developed a fast semi-quantitative method to analyse 41 chemical elements in wine, employing inductively coupled plasma mass spectrometry (ICP-MS). This methodology characterises what we term the mineral wine profile (MWP). In contrast to an organic molecular profile, the mineral composition of a wine remains constant from the moment it is bottled. Mineral elements play a crucial role in the terroir of wine: they pass primarily from soil to grape and are then influenced by various vinification techniques. Indeed, it is widely recognised that the original soil characteristics are altered by a multitude of winemaking procedures, presenting a considerable challenge when endeavouring to extract origin-related information in a typical scenario. Our study demonstrates that statistical analyses and artificial intelligence (AI) could be a tool for accurately deciphering origin information within the MWP, provided sufficient mineral elements are measured and a comprehensive database of wine samples is employed to establish effective learning. In this study, a dataset comprising 12,966 MWPs was created in just over a year. The first analysis revealed correlations between the elements in wine, especially between rare earth elements, between macronutrients and between micronutrients. A machine learning method was then developed to assess wine origin and principal grape variety. Six models were tested by comparing the area under the receiver operating characteristic curve (AUC), with eXtreme Gradient Boosting as the chosen model. Mean accuracies of 92 % for country classification, 91 % for the French wine region, and 85 % for the main grape variety were obtained, and mean AUC scores of 0.964 for country classification, 0.961 for the French wine region and 0.914 for the main grape variety. This study represents the first comprehensive investigation at this scale on wine samples, and underscores the importance of using a comprehensive MWP dataset for AI applications when verifying wine origin. The authentication of a wine with over 99 % specificity could be routinely achievable through this approach.
