Résumé : BACKGROUND: A number of studies have identified advanced age as a barrier to accessing specialised oncological care. Many factors can influence the care provided for elderly patients after a diagnosis of cancer has been established or is suspected. Only one European study has analysed the decision processes leading general practitioners (GPs) to refer elderly patients with cancer to oncologists. The objectives of the current study are to describe the factors that influence these decisions and to identify the particular factors and GP characteristics that are associated with systematic referral of these patients in South-West France. METHODS: This is a cross-sectional study on a representative sample of GPs in Aquitaine, South-West France. Questionnaire items were selected using a Delphi consensus approach and sent by post. Two logistic regression models were constructed to investigate GPs' decisions to refer these patients. RESULTS: The response rate obtained was 30%. Half of the general practitioners reported "always" referring their elderly cancer patients to oncologists. More than 75% reported being influenced by patient-related elements (patient and/or family wishes, comorbid factors, unsuitability of invasive investigations, physical and mental autonomy), by cancer-related elements (severity of symptoms, expected side-effects) and an organisational element (whether the general practitioner was used to collaborating with oncologists). Logistic regression analysis showed that cancer site and organisational difficulties in patient management were significantly associated with the decision to refer elderly patients with early-stage cancer. For advanced stages, oncology training, patient age, organisational difficulties in patient management and stage of cancer were significantly associated with the decision to refer elderly patients. CONCLUSIONS: Cancer-linked factors and organisational difficulties have been highlighted as influencing the decisions of GPs in the referral of elderly patients to a cancer team. These results highlight the need to implement continuous medical education specific for the management of elderly patients, to better apprehend the nature of these difficulties and to suggest solutions suited to local settings.

Résumé : Single-wall carbon nanotubes non-covalently functionalized with porphyrin molecules have proven to be a very promising light harvesting system either for energy or charge transfer. In this paper we investigate the dynamics of this coupling at a sub-picosecond time-scale, by means of transient absorption spectroscopy. We show that the ground state recovery time of the porphyrin is reduced by several orders of magnitude in the compound compared to the case of pristine porphyrin. Concomitantly, a strong bleaching signal is observed on the optical resonances of the nanotubes showing an ultrafast population buildup upon excitation of the porphyrin. We conclude that the energy transfer occurs on a time-scale shorter than 100 fs. Two-color measurements show that higher excited states of the nanotubes are populated on the same time-scale raising the point of the transfer mechanism. We briefly discuss two possible mechanisms.

Résumé : We review the African Monsoon Multidisciplinary Analysis (AMMA) model inter-comparison activities for West Africa. The Model Inter-comparison Project is an evaluation exercise of how global and regional atmospheric models represent seasonal and intra-seasonal variations of the climate and rainfall over the Sahel. The Land surface Model Inter-comparison Project in turn focuses on modelling critical land surface processes over West Africa and on their link with the atmosphere. The CHEmistry Model Inter-comparison Project (CHEMIP) is a comparison of the tropospheric composition as simulated by a number of Chemical Transport Models (CTM) and Chemistry-Climate Models. We highlight the main model limitations and provide recommendations for future development.

Résumé : An experiment was designed to observe a spreading phenomenon in the blogosphere. This experiment relied on a small applet that participants copied onto their own web page. The dataset obtained which is freely available for study is presented, and a basic analysis is conducted. It is discovered that, despite the classical assumption, in this experiment famous blogs do not necessarily act as super spreaders.

Résumé : Images obtained by the Optical, Spectroscopic, and Infrared Remote Imaging System (OSIRIS) cameras onboard the Rosetta spacecraft reveal that asteroid 21 Lutetia has a complex geology and one of the highest asteroid densities measured so far, 3.4 ± 0.3 grams per cubic centimeter. The north pole region is covered by a thick layer of regolith, which is seen to flow in major landslides associated with albedo variation. Its geologically complex surface, ancient surface age, and high density suggest that Lutetia is most likely a primordial planetesimal. This contrasts with smaller asteroids visited by previous spacecraft, which are probably shattered bodies, fragments of larger parents, or reaccumulated rubble piles.

Résumé : Mentioned for the first time in 1831, the ancient aqueduct of Reims has been the subject of various observations during the 19th century, before it’s been erased for memory with world war one. It was only at the beginning of the eighties that the G.E.A.C.A. (1) undertook a new study of the building by means of prospection campaigns, backed by borings. From 2005, preventive archaeological operations of wider scale have been achieved in the south-east periphery of Reims. These recent research allowed to improve the architectural knowledge of the hydraulic work and the precision of its layout of close to 25 miles. It was possible to answer questions largely unknown so far such as chronology or materials recovery methods. According to results from ceramics analysis and radiocarbon dating, it is now proven that the construction of the work took place in the second half of Augustus reign, in the first quarter of the first century of our era. A higher accuracy would be required regarding when it was stopped to be used but the correlation of archaeological items and data established from the ancient hydraulic fitting-out inside the city seem to indicate the end of the 4th century or the beginning of the 5th century. The first salvage of materials from the aqueduct is prior to the Merovigien period. The remains surrounding the work show that the demolition of the fittings required an important and well organized means. Other materials salvage operations will follow one another until the 19th century. The synthesis of the whole archaeological work built up from the re-discovery of the monument, in the years of 1830, to nowadays is presented in this book. (traduction : Fabrice Laporta)
