Résumé : Uncertainties in model projections of future climate change are high, and have not decreased substantially over the last 30 years: IPCC AR4 range of climate sensitivity - the increase in globally averaged surface temperature expected for a doubling of CO2 - is [1.5 °C, 4.5 °C]. The causes and nature of this uncertainty, more specifically whether or not it will be possible to reduce it, is subject to debate. Before digging into this problem, we start by focusing on a basic, somewhat epistemological question: what is uncertainty in the context of climate science, and how should it be quantified? While this question may at first appear trivial, we claim that it has occasionally proved to be a source of confusion in the aforementioned debate. We illustrate this point by revisiting one among the most significant contributions to this debate in the recent past (Roe and Baker, 2007) and highlight the fact that its conclusions are dramatically affected by the chosen definition of uncertainty. To resolve this issue, we propose widely used standard deviation as a unique, broadly applicable definition of uncertainty. We then recall a general result on the propagation of uncertainty available from Probability theory under this definition, and analyse its implication on a simple stochastic Climate toy model. This analysis suggests that high uncertainty is not an inevitable and general consequence of the nature of the climate system, and hence is not doomed to remain high. More specifically, it suggests that reducing uncertainty on feedbacks in GCMs, through an improved understanding of involved physical processes, does lead to a reduction of uncertainty on climate sensitivity projected by models. Finally, we elaborate on foreseeable advances in Climate research and modeling that may lead to a decrease of feedback uncertainty, and subsequently to a reduction of uncertainty on climate sensitivity in the near future.

Résumé : We report the genotyping analysis of Toxoplasma gondii isolates in samples collected from 88 immunocompromised patients, along with clinical and epidemiological data. Most of these samples were collected in France during the current decade by the Toxoplasma Biological Resource Center. Lack of specific anti-Toxoplasma treatment, pulmonary toxoplasmosis, and involvement of multiple organs were the 3 main risk factors associated with death for this patient group. Genotyping results with 6 microsatellite markers showed that type II isolates were predominant among patients who acquired toxoplasmic infection in Europe. Non-type II isolates included 13 different genotypes and were mainly collected from patients who acquired toxoplasmosis outside Europe. Type III was the second most common genotype recovered from patients, whereas type I was rare in our population. Three nonarchetypal genotypes were repeatedly recovered from different patients who acquired the infection in sub-Saharan Africa (genotypes Africa 1 and Africa 2) and in the French West Indies (genotype Caribbean 1). The distribution of genotypes (type II vs. non-type II) was not significantly different when patients were stratified by underlying cause of immunosuppression, site of infection, or outcome. We conclude that in immunocompromised patients, host factors are much more involved than parasite factors in patients' resistance or susceptibility to toxoplasmosis.

Résumé : PURPOSE: To determine event free and overall survival, and long-term cognitive sequelae of children with standard-risk medulloblastoma (SRM) treated with hyperfractionated radiotherapy, conformal reduced boost volume without chemotherapy, and online quality assurance. PATIENTS AND METHODS: Forty-eight patients (age 5 to 18 years) were included in the Medulloblastoma-Société Française d'Oncologie Pédiatrique (MSFOP 98) protocol (December 1998 to October 2001). Patients received hyperfractionated radiotherapy (HFRT; 36 Gy, 1 Gy/fraction twice per day) to the craniospinal axis followed by a boost to the tumor bed (1.5-cm margin) to a dose of 68 Gy. Records of craniospinal irradiation were reviewed before treatment started. Neuropsychologic evaluations were done according to the protocol (1, 3, 5, and 7 years after irradiation). Cognitive outcomes were followed longitudinally with full-scale intelligence quotient (FSIQ) obtained with age-adapted Wechsler scales. RESULTS: After a median follow-up of 77.7 months, 6-year overall survival (OS) and event-free survival (EFS) rates for the cohort were 78% (95% CI, 66% to 90%) and 75%, respectively (95% CI, 62% to 87%). Thanks to quality control, 14 major deviations were detected. Annual full scale IQ decline was 2 points over a 6-year period. Predicted change in FSIQ points per year was 2.15 (95% CI, -1.24 to 3.51) with an intercept (ie, predicted FSIQ) of 93.57 at baseline. CONCLUSION: HFRT protocol with conformal reduced boost and online quality control allows excellent long-term OS and EFS in the absence of chemotherapy. In addition, FSIQ drops seem to be less pronounced than previously reported with standard irradiation regimens.

Résumé : Conditions for the elaboration of nanostructured varistors by Spark Plasma Sintering (SPS) are investigated, using 8 nm zinc oxide nanoparticles synthesized following an organometallic approach. A binary system constituted of zinc oxide and bismuth oxides nanoparticles is used for this purpose. It is synthesized at room temperature in an organic solution through the hydrolysis of dicyclohexylzinc and bismuth acetate precursors. Sintering of this material is performed by SPS at various temperatures and dwell times. The determination of the microstructure and the chemical composition of the as prepared ceramics are based on Scanning Electron Microscopy (SEM) and X-Ray Diffraction (XRD) analysis. The non linear electrical characteristics are evidenced by current-voltage (I-V) measurements. The breakdown voltage of these nanostructured varistors strongly depends on grain sizes. The results show for the first time that, nanostructured varistors are obtained by SPS at sintering temperatures ranging from 550 to 600°C.

Résumé : We describe what we believe to be the first documented case of Mycobacterium goodii infection in Europe. It is also the second documented report of a pacemaker pocket surgical site infection caused by M. goodii . Although rarely involved in such infections, rapidly growing mycobacteria should be recognized during conventional bacteriological investigations and further identified by molecular tools to provide adequate therapy. In the present case, antimicrobial therapy with doxycycline without removal of the pacemaker was successful.
