Résumé : OBJECTIVE: Associations between metabolic syndrome and its individual components with risk of incident dementia and its different subtypes are inconsistent. RESEARCH DESIGN AND METHODS: The 7,087 community-dwelling subjects aged > or =65 years were recruited from the French Three-City (3C) cohort. Hazard ratios (over 4 years) of incident dementia and its subtypes (vascular dementia and Alzheimer's disease) and association with metabolic syndrome (defined according to the National Cholesterol Education Program Adult Treatment Panel III criteria) and its individual components (hypertension, large waist circumference, high triglycerides, low HDL cholesterol, and elevated fasting glycemia) were estimated in separate Cox proportional hazard models. RESULTS: Metabolic syndrome was present in 15.8% of the study participants. The presence of metabolic syndrome increased the risk of incident vascular dementia but not Alzheimer's disease over 4 years, independent of sociodemographic characteristics and the apolipoprotein (apo) Eepsilon4 allele. High triglyceride level was the only component of metabolic syndrome that was significantly associated with the incidence of all-cause (hazard ratio 1.45 [95% CI 1.05-2.00]; P = 0.02) and vascular (2.27 [1.16-4.42]; P = 0.02) dementia, even after adjustment of the apoE genotype. Diabetes, but not impaired fasting glycemia, was significantly associated with all-cause (1.58 [1.05-2.38]; P = 0.03) and vascular (2.53 [1.15-5.66]; P = 0.03) dementia. CONCLUSIONS: The observed relation between high triglycerides, diabetes, and vascular dementia emphasizes the need for detection and treatment of vascular risk factors in older individuals in order to prevent the likelihood of clinical dementia.

Résumé : OBJECTIVE: Osteopontin (OPN) plays an important role in the development of insulin resistance and liver complications in dietary murine models. We aimed to determine the expression pattern of OPN and its receptor CD44 in obese patients and mice according to insulin resistance and liver steatosis. RESEARCH DESIGN AND METHODS: OPN and CD44 expressions were studied in 52 morbidly obese patients and in mice. Cellular studies were performed in HepG2 cells. RESULTS: Hepatic OPN and CD44 expressions were strongly correlated with liver steatosis and insulin resistance in obese patients and mice. This increased OPN expression could be due to the accumulation of triglycerides, since fat loading in HepG2 promotes OPN expression. In contrast, OPN expression in adipose tissue (AT) was enhanced independently of insulin resistance and hepatic steatosis in obese patients. The elevated OPN expression in AT was paralleled with the AT macrophage infiltration, and both phenomena were reversed after weight loss. The circulating OPN level was slightly elevated in obese patients and was not related to liver steatosis. Further, AT did not appear to secrete OPN. In contrast, bariatric surgery-induced weight loss induced a strong increase in circulating OPN. CONCLUSIONS: The modestly elevated circulating OPN levels in morbidly obese patients were not related to liver steatosis and did not appear to result from adipose tissue secretion. In subcutaneous AT, expression of OPN was directly related to macrophage accumulation independently from liver complications. In contrast, hepatic OPN and CD44 expressions were related to insulin resistance and steatosis, suggesting their local implication in the progression of liver injury.

Résumé : The advent of TNF antagonists has had a revolutionary impact on the treatment of ankylosing spondylitis (AS). However, whether the benefits recorded in controlled trials are replicated in everyday practice has not been extensively evaluated. Objectives: To evaluate the effectiveness of infliximab in patients with axial spondyloarthropathies, to identify factors associated with the treatment response, and to assess fulfillment of modified New York criteria and compliance with guidelines about using TNF antagonists. Methods: Retrospective review of patients given infliximab for axial spondyloarthropathy between 2001 and 2003. Disease activity, motion limitation, laboratory tests, and the 6-week response rate were recorded. Results: Of the 86 included patients (48 women and 38 men, mean age, 44 +/- 11 years), 37% responded to infliximab therapy. Uveitis and C-reactive protein elevation at baseline predicted a response. Only 53% of patients met modified New York criteria and only 23% met ASAS criteria for starting TNF antagonist therapy. Conclusion: Infliximab was less effective in our patients with axial spondyloarthropathy than expected based on the results of controlled trials. However, many patients did not meet New York criteria for AS and/or ASAS criteria for TNF antagonist therapy. Therefore, our results do not challenge the usefulness of TNF antagonists in axial AS. Our patients were treated before the development of guidelines for TNF antagonist therapy and before the introduction of magnetic resonance imaging as an evaluation tool in AS. Strict criteria should be used to decide when TNF antagonist therapy is appropriate in patients with spondyloarthropathies. (C) 2008 Elsevier Masson SAS. All rights reserved.

Résumé : During the ZaïRov2 cruise of the ZaïAngo project (1998-2000) on the passive Congo-Angola margin, several gravity cores were analyzed for helium isotopic composition of sedimentary pore waters in two cold fluid seepage zones: the Astrid slide area and the Regab giant pockmark. Gas concentration and isotopic composition are presented along with thermal data in terms of the origin and circulation of fluids. Helium isotope data lie on a mixing line between bottom seawater and an almost pure radiogenic. Helium and temperature vertical profiles are well described by the classic diffusion-advection equation. On the basis of He profiles, we estimate the advection rate on the rim of the pockmark between 1.2 and 2.3 mm/a. The He flux derived for a pure diffusive regime (2.4 Â 10 À8 mol/m 2 /a) can favorably be compared to literature data and contrasts with the flux computed close to the pockmark center (1.9 Â 10 À7 mol/m 2 /a). Helium depth profiles turned to be more sensitive to advection rate than temperature profiles are.

Résumé : The Vasco–Cirene program explores how strong air–sea interactions promoted by the shallow thermocline and high sea surface temperature in the Seychelles–Chagos thermocline ridge results in marked variability at synoptic, intraseasonal, and interannual time scales. The Cirene oceanographic cruise collected oceanic, atmospheric, and air–sea flux observations in this region in January–February 2007. The contemporaneous Vasco field experiment complemented these measurements with balloon deployments from the Seychelles. Cirene also contributed to the development of the Indian Ocean observing system via deployment of a moor-ing and 12 Argo profilers.Unusual conditions prevailed in the Indian Ocean during January and February 2007, following the Indian Ocean dipole climate anomaly of late 2006. Cirene measurements show that the Seychelles–Chagos thermocline ridge had higher-than-usual heat content with subsurface anomalies up to 7°C. The ocean surface was warmer and fresher than average, and unusual eastward currents prevailed down to 800 m. These anomalous conditions had a major impact on tuna fishing in early 2007. Our dataset also sampled the genesis and maturation of Tropical Cyclone Dora, including high surface temperatures and a strong diurnal cycle before the cyclone, followed by a 1.5°C cooling over 10 days. Balloonborne instruments sampled the surface and boundary layer dynamics of Dora. We observed small-scale structures like dry-air layers in the atmosphere and diurnal warm layers in the near-surface ocean. The Cirene data will quantify the impact of these finescale features on the upper-ocean heat budget and atmospheric deep convection.

Résumé : This paper presents an analysis that searched for systematic effects within the CoRoT exoplanet field light curves. The analysis identified a systematic effect that modified the zero point of most CoRoT exposures as a function of stellar magnitude. We could find this effect only after preparing a set of learning light curves that were relatively free of stellar and instrumental noise. Correcting for this effect, rejecting outliers that appear in almost every exposure, and applying SysRem, reduced the stellar RMS by about 20%, without attenuating transit signals.

Résumé : The burial-exhumation cycle of crustal material in subduction zones can either be driven by the buoyancy of the material, by the surrounding flow, or by both. High pressure and ultrahigh pressure rocks are chiefly exhumed where subduction zones display transient behaviours, which lead to contrasted flow regimes in the subduction mantle wedge. Subduction zones with stationary trenches (mode I) favour the burial of rock units, whereas slab rollback (mode II) moderately induces an upward flow that contributes to the exhumation, a regime that is reinforced when slab dip decreases (mode III). Episodic regimes of subduction that involve different lithospheric units successively activate all three modes and thus greatly favour the exhumation of rock units from mantle depth to the surface without need for fast and sustained erosion.
