Résumé : Objectives: The aims of this study were (i) to investigate the population pharmacokinetics of tacrolimus in renal transplant recipients, including the influence of biological and pharmacogenetic covariates; and (ii) to develop a Bayesian estimator able to reliably estimate the individual pharmacokinetic parameters and inter-dose area under the blood concentration-time curve (AUC) from 0 to 12 hours (AUC12) in renal transplant patients. Methods: Full pharmacokinetic profiles were obtained from 32 renal transplant patients at weeks 1 and 2, and at months 1, 3 and 6 post-transplantation. The population pharmacokinetic analysis was performed using the nonlinear mixed-effect modelling software NONMEM® version VI. Patients’ genotypes were characterized by allelic discrimination for PXR −25385C>T genes. Results: Tacrolimus pharmacokinetics were well described by a two-compartment model combined with an Erlang distribution to describe the absorption phase, with low additive and proportional residual errors of 1.6 ng/mL and 9%, respectively. Both the haematocrit and PXR −25385C>T single nucleotide polymorphism (SNP) were identified as significant covariates for apparent oral clearance (CL/F) of tacrolimus, which allowed improvement of prediction accuracy. Specifically, CL/F decreased gradually with the number of mutated alleles for the PXR −25385C>T SNP and was inversely proportional to the haematocrit value. However, clinical criteria of relevance, mainly the decrease in interindividual variability and residual error, led us to retain only the haematocrit in the final model. Maximum a posteriori Bayesian forecasting allowed accurate prediction of the tacrolimus AUC12 using only three sampling times (at 0 hour [predose] and at 1 and 3 hours postdose) in addition to the haematocrit value, with a nonsignificant mean AUC bias of 2% and good precision (relative mean square error = 11%). Conclusion: Population pharmacokinetic analysis of tacrolimus in renal transplant recipients showed a significant influence of the haematocrit on its CL/F and led to the development of a Bayesian estimator compatible with clinical practice and able to accurately predict tacrolimus individual pharmacokinetic parameters and the AUC12.

Résumé : OBJECTIVE: To assess the occurrence of weight gain in patients with Parkinson's disease, with an average 16 months of follow-up after subthalamic nucleus deep brain stimulation. METHODS: We used dual x ray absorptiometry to evaluate changes in body weight and body composition in 22 patients with Parkinson's disease (15 men and seven women) before surgery, 3 months after surgery and on average 16 months after surgery. RESULTS: No patient was underweight before surgery and 50% were overweight. By contrast, 68% were overweight or obese 3 months after surgery and 82% after 16 months (p<0.001). For men, the mean increase in body mass index (BMI) was 1.14 (0.23) kg/m(2) 3 months after surgery and 2.02 (0.36) kg/m(2) 16 months after surgery. For women, the mean increases in BMI at the same evaluation times were 1.04 (0.30) kg/m(2) and 2.11 (0.49) kg/m(2). This weight gain was mainly secondary to an increase in fat mass in both men and women. Three months after surgery, acute subthalamic deep brain stimulation induced an improvement in parkinsonian symptoms (evaluated by the Unified Parkinson Disease Rating Scale (UPDRS) part III) by 60.7 (2.9)% in the "off" dopa condition and a dramatic improvement of motor complications (dyskinesia duration: 82.8 (12.8)%, p<0.0001; off period duration: 92.7 (18.8)%, p<0.0001). CONCLUSION: Although subthalamic nucleus deep brain stimulation significantly improved parkinsonian symptoms and motor complications, many patients became overweight or obese. This finding highlights the necessity to understand the underlying mechanisms and to provide a diet management with a physical training schedule appropriate for patients with Parkinson's disease.

Résumé : We present airborne measurements of carbon dioxide (CO2), carbon monoxide (CO), ozone (O3), equivalent black carbon (EBC) and ultra fine particles over North-Eastern Siberia in July 2008 performed during the YAK-AEROSIB/POLARCAT experiment. During a "golden day" (11 July 2008) a number of biomass burning plumes were encountered with CO concentration enhancements of up to 500 ppb relative to a background of 90 ppb. Number concentrations of aerosols in the size range 3.5–200 nm peaked at 4000 cm−3 and the EBC content reached 1.4 μg m−3. These high concentrations were caused by forest fires in the vicinity of the landing airport in Yakutsk where during the descent measurements in fresh smoke could be made. We estimate a combustion efficiency of 90±3% based on CO and CO2 measurements. The emission factor of CO emitted was 59.6±15.2 g CO per kilogram of dry matter burned, suggesting an increase in the average northern hemispheric CO concentration of 3.0–7.2 ppb per million hectares of Siberian forest burned. For BC, we estimate an emission factor of 0.52±0.07 g BC kg−1, comparable to values reported in the literature. The emission ratio of ultra-fine particles (3.5–200 nm) was 26 cm−3 (ppb CO)−1, consistent with other airborne studies.

Résumé : Les biologistes s’attachent actuellement à prédire la fonction des gènes d’organismes de génome séquence à partir de données transcriptomes, issues de l’utilisation des puces à ADN. Le d´développement de cette technologie permet de tester l’expression de l’ensemble du génome dans de nombreuses conditions expérimentales. Cette quantité d’information peut alors sembler être un atout pour la classification des gènes. Pourtant il est courant que seul un sous-ensemble contienne l’information pertinente pour la classification. Les procédures de sélection des variables en classification non supervisée par mélanges gaussiens supposent généralement que les variables non informatives pour la classification sont soit toutes indépendantes, soit liées à des variables informatives. Nous proposons une nouvelle modélisation du rôle des variables plus polyvalente : les variables sont soit informatives pour la classification, soit redondantes, soit totalement indépendantes. Nous proposons un critère de sélection des variables et un algorithme pour cette nouvelle modélisation. L’intérêt de cette nouvelle modélisation pour la prédiction de la fonction des gènes orphelins est illustrée sur un ensemble de données transcriptomes obtenues chez Arabidopsis thaliana.

Résumé : A biophysical model of receptor potential generation in the male moth olfactory receptor neuron is presented. It takes into account all pre-effector processes-the translocation of pheromone molecules from air to sensillum lymph, their deactivation and interaction with the receptors, and the G-protein and effector enzyme activation-and focuses on the main post-effector processes. These processes involve the production and degradation of second messengers (IP3 and DAG), the opening and closing of a series of ionic channels (IP3-gated Ca2+ channel, DAG-gated cationic channel, Ca2+-gated Cl- channel, and Ca2+-and voltage-gated K+ channel), and Ca2+ extrusion mechanisms. The whole network is regulated by modulators (protein kinase C and Ca2+-calmodulin) that exert feedback inhibition on the effector and channels. The evolution in time of these linked chemical species and currents and the resulting membrane potentials in response to single pulse stimulation of various intensities were simulated. The unknown parameter values were fitted by comparison to the amplitude and temporal characteristics (rising and falling times) of the experimentally measured receptor potential at various pheromone doses. The model obtained captures the main features of the dose-response curves: the wide dynamic range of six decades with the same amplitudes as the experimental data, the short rising time, and the long falling time. It also reproduces the second messenger kinetics. It suggests that the two main types of depolarizing ionic channels play different roles at low and high pheromone concentrations; the DAG-gated cationic channel plays the major role for depolarization at low concentrations, and the Ca2+-gated Cl- channel plays the major role for depolarization at middle and high concentrations. Several testable predictions are proposed, and future developments are discussed.

Résumé : Purpose: : To correlate the presenting clinical ophthalmic features with the bacterial identification in 100 patients with acute post catarcat endophthalmitis enrolled in the FRench Instutionnal ENdophthalmitis Study (FRIENDS). Methods: : Demographic data, past medical history and initial eye examination were recorded in a standardized form in 100 patients with acute endophthalmitis after cataract extraction (< 6 weeks) in a prospective multicenter study. Relationship between microbiological identification (using conventional cultures and panbacterial PCR) and clinical factors at baseline was studied using univariate (ANOVA). Results: : 100 patients were hospitalized for endophthalmitis treatment with a mean delay of 8±7.5 (SD) days (range: 2-31 days) after the cataract surgery, complicated by capsular rupture in 19% of the cases. Main symptoms were loss of vision (95%) frequently limited to light perception (32%), and pain (75%). More frequent clinical signs were hypopion (72%), cyclitic membrane (77.5%) and absence of fundus visibility (90%). As compared with Staphylococcus species, the identification of Streptococcus species was more frequently identified (p<0.05) in male and diabetic patients, with initial visual acuity limited to light perceptions and a painfull eye. The Staphylococcus aureus and lugdunensis group differed from other coagulase negative Staphylococcus group (p<0.05) for the height of hypopion. Conclusions: : The baseline features of acute endophthalmitis after cataract surgery in the era of phacoemulsification are similar to those reported in the EVS 10 years ago. The presenting characteristics of acute post cataract endophthalmitis differ according to the infecting organism. The association between clinical factors at baseline and bacteria identified using cultures and/or eubacterial PCR does not appear sufficiently predictive to guide empiric intraocular antibiotic therapy. However our results suggest that other baseline characteristics than initial visual acuity may be useful to detect presumed infected patients with a virulent species needing a more aggressive therapeutic approach.

Résumé : Le présent volume consacré à l’habitat pyrénéen au cours des périodes médiévale et moderne est le fruit de la collaboration de plusieurs universités espagnoles (universités du Pays Basque-UPV ; publique de Navarre-UPN ; de Saragosse, de Gérone) et françaises (universités de Perpignan Via domitia, de Toulouse-Le Mirail, de Pau et des Pays de l’Adour). C’est en effet au sein de RESOPYR, un réseau d’universités franco-espagnoles réunies autour d’un projet commun élaboré dans le cadre de la Communauté de travail des Pyrénées (CTP), qu’il a été préparé. Il regroupe une vingtaine de contributions concernant la montage pyrénéenne et son piémont, de l’Atlantique à la Méditerranée, depuis le Moyen Âge jusqu’à nos jours. Ce volume aborde les problèmes de l’habitat et s’organise autour de trois cadres principaux ; certains articles traitent de la naissance et de la mise en place des villages ; d’autres envisagent les différentes modes de hiérarchisation et de fonctionnement de l’habitat dans un modèle communautaire qui n’est jamais égalitaire ; enfin, les derniers analysent la remise en cause de ce modèle communautaire et sa rupture définitive au profit d’un accaparement individuel.
