Résumé : BACKGROUND: Isolated posterior leaflet mitral valve prolapse (PostMVP), a common form of MVP, often referred as fibroelastic deficiency, is considered a degenerative disease. PostMVP patients are usually asymptomatic and often undiagnosed until chordal rupture. The present study aims to characterize familial PostMVP phenotype and familial recurrence, its genetic background, and the pathophysiological processes involved.METHODS: We prospectively enrolled 284 unrelated MVP probands, of whom 178 (63%) had bi-leaflet MVP and 106 had PostMVP (37%). Familial screening within PostMVP patients allowed the identification of 20 families with inherited forms of PostMVP for whom whole genome sequencing was carried out in probands. Functional in vivo and in vitro investigations were performed in zebrafishand in Hek293T cells.RESULTS: In the 20 families with inherited form of PostMVP, 38.8% of relatives had a MVP/prodromal form, mainly of the posterior leaflet, with transmission consistent with an autosomal dominant mode of inheritance. Compared with control relatives, PostMVP family patients have clear posterior leaflet dystrophy on echocardiography. Patients with PostMVP present a burden of rare genetic variants in ARHGAP24. ARHGAP24 encodes the filamin A binding RhoGTPase-activating protein FilGAP and its silencing in zebrafish leads to atrioventricular regurgitation. In vitro functional studies showed that variants of FilGAP, found in PostMVP families, are loss-of-function variants impairing cellular adhesion and mechano-transduction capacities. CONCLUSIONS PostMVP should not only be considered an isolated degenerative pathology but as a specific heritable phenotypic trait with genetic and functional pathophysiological origins. The identification of loss-of-function variants in ARHGAP24 further reinforces the pivotal role of mechano-transduction pathways in the pathogenesis of MVP.CLINICAL PERSPECTIVE Isolated posterior mitral valve prolapse (PostMVP), often called fibro-elastic deficiency MVP, is at least in some patients, a specific inherited phenotypic trait PostMVP has both genetic and functional pathophysiological origins Genetic variants in the ARHGAP24 gene, which encodes for the FilGAP protein, cause progressive Post MVP in familial cases, and impair cell adhesion and mechano-transduction capacities

Résumé : Background Nodes are the second site for prostate cancer recurrence. Whole-pelvic radiotherapy (WPRT) has shown superiority over nodal stereotactic body radiotherapy (SBRT) in two retrospective cohorts. We aimed to compare both modalities and assess factors associated with treatment outcomes. Materials and methods This retrospective multicentric cohort study included patients from five institutions spanning from 2010 to 2022. Patients had a history of prostatic adenocarcinoma classified as N0 M0 at diagnosis with a first nodal-only pelvic castration-sensitive recurrence. Failure-free survival (FFS) was defined as the time from the end of RT to the first failure event–biochemical or imaging recurrence, or death. Results A total of 147 patients (pts) were analyzed, mainly treated for a recurrence after initial prostatectomy (87%), with 64 (43.5%) undergoing SBRT and 83 (56.5%) undergoing WPRT. SBRT was chosen mainly for dosimetric constraints (67%) and was associated with a lower rate of concomitant androgen deprivation therapy (ADT) prescription. With a median follow-up of 68 months [inter-quartile range (IQR) = 51], FFS was significantly lower in the SBRT group (p < 0.0001). In multivariable analysis, WPRT and ADT were associated with a longer FFS. Factors associated with a longer FFS after SBRT included associated ADT, lower prostate-specific antigen (PSA) levels, a PSA doubling time >6 months, and a Gleason score <8. SBRT was associated with a lower rate of genitourinary and gastrointestinal grade ≥2 complications. Discussion For an isolated pelvic nodal prostate cancer recurrence, SBRT is associated with a shorter FFS compared to WPRT. SBRT is often more convenient for patients and leaves further pelvic salvage options available, so it can be explored as an option for well-informed patients.

Résumé : For the wine industry, ensuring quality and authenticity hinges on the precise determination of wine origin. In our study, we developed a fast semi-quantitative method to analyse 41 chemical elements in wine, employing inductively coupled plasma mass spectrometry (ICP-MS). This methodology characterises what we term the mineral wine profile (MWP). In contrast to an organic molecular profile, the mineral composition of a wine remains constant from the moment it is bottled. Mineral elements play a crucial role in the terroir of wine: they pass primarily from soil to grape and are then influenced by various vinification techniques. Indeed, it is widely recognised that the original soil characteristics are altered by a multitude of winemaking procedures, presenting a considerable challenge when endeavouring to extract origin-related information in a typical scenario. Our study demonstrates that statistical analyses and artificial intelligence (AI) could be a tool for accurately deciphering origin information within the MWP, provided sufficient mineral elements are measured and a comprehensive database of wine samples is employed to establish effective learning. In this study, a dataset comprising 12,966 MWPs was created in just over a year. The first analysis revealed correlations between the elements in wine, especially between rare earth elements, between macronutrients and between micronutrients. A machine learning method was then developed to assess wine origin and principal grape variety. Six models were tested by comparing the area under the receiver operating characteristic curve (AUC), with eXtreme Gradient Boosting as the chosen model. Mean accuracies of 92 % for country classification, 91 % for the French wine region, and 85 % for the main grape variety were obtained, and mean AUC scores of 0.964 for country classification, 0.961 for the French wine region and 0.914 for the main grape variety. This study represents the first comprehensive investigation at this scale on wine samples, and underscores the importance of using a comprehensive MWP dataset for AI applications when verifying wine origin. The authentication of a wine with over 99 % specificity could be routinely achievable through this approach.

La nomenclature proposée a immédiatement été baptisée « Nomenclature Dintilhac » et a été très rapidement adoptée par les acteurs du droit du dommage corporel. Philippe Brun et Christophe Quézel-Ambrunaz conversent sur les presque vingt ans d’utilisation de celle-ci.

Résumé : Oxygenators, alarm devices, and footsteps are some of the most common sound sources in a hospital. Detecting them has scientific value for environmental psychology but comes with challenges of its own: namely, privacy preservation and limited labeled data. In this paper, we address these two challenges via a combination of edge computing and cloud computing. For privacy preservation, we have designed an acoustic sensor which computes third-octave spectrograms on the fly instead of recording audio waveforms. For sample-efficient machine learning, we have repurposed a pretrained audio neural network (PANN) via spectral transcoding and label space adaptation. A small-scale study in a neonatological intensive care unit (NICU) confirms that the time series of detected events align with another modality of measurement: i.e., electronic badges for parents and healthcare professionals. Hence, this paper demonstrates the feasibility of polyphonic machine listening in a hospital ward while guaranteeing privacy by design.

Résumé : Background: Any surgical procedure carries a risk for venous thromboembolism (VTE), albeit variable. Improvements in medical and surgical practices and the shortening of care pathways due to the development of day surgery and enhanced recovery after surgery, have reduced the perioperative risk for VTE. Objective: A collaborative working group of experts in perioperative haemostasis updated in 2024 the recommendations for the Prevention of perioperative venous thromboembolism published in 2011. Methods: The addressed questions were defined by 40 experts (GIHP, SFAR, SFTH and SFMV) and formulated in a PICO format. They performed the literature review and formulated recommendations according to the Grading of GRADE system. Recommendations were then validated by a vote determining the strength of each recommendation. Of note, these recommendations do not cover all surgical specialties. Especially, thromboprophylaxis in cardiac surgery, neurosurgery and obstetrics is not addressed. Results: 78 recommendations were formalized into 17 sections, including patient-related VTE risk factors, types of surgery, extreme body weight, renal impairment, mechanical prophylaxis, distal deep vein thrombosis; 27 were found to have a high level of evidence (GRADE 1) and 41 a low level of evidence (GRADE 2) and 10 were expert opinion. All had strong agreement among the experts. Conclusions: These guidelines help to weigh the perioperative risk for VTE (which includes the risk associated to surgery and the patient-related risk) against the adverse effects of thromboprophylaxis, either pharmacological or mechanical. This includes particularly the bleeding risk induced by antithrombotic drugs as well as costs.

Résumé : African researchers continue to face numerous barriers that limit the global dissemination and advancement of their research. These include high open-access publication fees, requirements for publishing in high-impact journals, inability to attend or host international conferences, and limited access to published papers and resources. In this viewpoint, we focus on the specific challenge of attending international scientific conferences hosted in high-income settings and aim to propose practical solutions to address this inequity. The related barriers include visa-related difficulties and prohibitive travel expenses (e.g. non-reimbursable high visa fees, flight tickets, accommodation and subsistence, and conference registration costs).

Résumé : PURPOSE The use of inotuzumab ozogamicin (InO), a conjugated anti-CD22 monoclonal antibody, is becoming a promising frontline treatment for older patients with ALL. PATIENTS AND METHODS EWALL-INO is an open-label prospective multicenter phase II trial (ClinicalTrials.gov identifier: NCT03249870 ). Patients age 55 years and older with newly diagnosed CD22 + Philadelphia chromosome–negative (Ph–) B-cell precursor (BCP) ALL were eligible. After a prephase, a first induction consisting of vincristine, dexamethasone, and three injections of InO (0.8 mg/m 2 day 1, 0.5 mg/m 2 day 8/day 15) was followed by a second induction combining cyclophosphamide, dexamethasone, and two injections of InO (0.5 mg/m 2 day 1/day 8). Responders received up to six cycles of chemotherapy consolidation and 18-month chemotherapy maintenance. Allotransplant was allowed after three consolidations. The primary end point was 1-year overall survival (OS). RESULTS Between December 2017 and March 2022, 131 patients (median age 68 years) were included. Three patients died during induction 1 (n = 130), two from multiple organ failure and one from hemorrhage, and none during induction 2 (n = 120). After induction 2, 90% of the patients achieved complete remission (CR) or CR with incomplete platelet recovery (CRp) and 80% had measurable residual disease (MRD2) <10 −4 . Among responders (n = 119), 47 relapsed and 14 died in CR/CRp. One-year OS, relapse-free survival (RFS), and cumulative incidence of relapse (CIR) rates were 73.2%, 66%, and 25%, respectively. High-risk cytogenetics and lower CD22 expression (<70%) were associated with worse OS, while both high-risk cytogenetics and MRD2 ≥10 −4 were associated with lower RFS and higher CIR. The 10 allotransplanted patients had very favorable outcomes (90% 2-year OS/RFS and no relapse). Only one nonfatal sinusoidal obstructive syndrome was documented during the study. CONCLUSION Our results support InO's use in first-line regimens for older patients with CD22 + Ph– BCP-ALL.
