Résumé : The French Society of Rheumatology recommendations for managing rheumatoid arthritis (RA) has been updated by a working group of 21 rheumatology experts, 4 young rheumatologists and 2 patient association representatives on the basis of the 2023 version of the European Alliance of Associations for Rheumatology (EULAR) recommendations and systematic literature reviews. Two additional topics were addressed: people at risk of RA development and RA-related interstitial lung disease (RA-ILD). Four general principles and 19 recommendations were issued. The general principles emphasize the importance of a shared decision between the rheumatologist and patient and the need for comprehensive management, both drug and non-drug, for people with RA or at risk of RA development. In terms of diagnosis, the recommendations stress the importance of clinical arthritis and in its absence, the risk factors for progression to RA. In terms of treatment, the recommendations incorporate recent data on the cardiovascular and neoplastic risk profile of Janus kinase inhibitors. With regard to RA-ILD, the recommendations highlight the importance of clinical screening and the need for high-resolution CT scan in the presence of pulmonary symptoms. RA-ILD management requires collaboration between rheumatologists and pulmonologists. The treatment strategy is based on controlling disease activity with methotrexate or targeted therapies (mainly abatacept or rituximab). The prescription for anti-fibrotic treatment should be discussed with a pulmonologist with expertise in RA-ILD.

Résumé : Introduction: First-line oxygenation strategy in patients with acute hypoxaemic respiratory failure consists in standard oxygen or high-flow nasal oxygen therapy. Clinical practice guidelines suggest the use of high-flow nasal oxygen rather than standard oxygen. However, findings remain contradictory with a low level of certainty. We hypothesise that compared with standard oxygen, high-flow nasal oxygen may reduce mortality in patients with acute hypoxaemic respiratory failure.
Method and analysis: The Standard Oxygen versus High-flow nasal Oxygen-trial is an investigator-initiated, multicentre, open-label, randomised controlled trial comparing high-flow nasal oxygen versus standard oxygen in patients admitted to an intensive care unit (ICU) for acute respiratory failure with moderate-to-severe hypoxaemia. 1110 patients will be randomly assigned to one of the two groups with a ratio of 1:1. The primary outcome is the number of patients who died 28 days after randomisation. Secondary outcomes include comfort, dyspnoea and oxygenation 1 hour after treatment initiation, the number of patients intubated at day 28, mortality in ICU, in hospital and until day 90, and complications during ICU stay.
Ethics and dissemination:The study has been approved by the central Ethics Committee 'Sud Méditerranée III' (2020-07-05) and patients will be included after informed consent. The results will be submitted for publication in peer-reviewed journals.

Résumé : We herein propose capitalizing on strong hydrogen bonding from novel bio-CO 2 -derived dynamic thermosets to achieve high-performance natural fiber composites (NFC) with circular features. CO 2 -and biomass-derived polyhydroxyurethane (PHU) thermosets were selected, for the first time of our knowledge, as matrices for their ability to make strong H-bond, resulting in outstanding mechanical properties for NFC. Exploiting this H-bond key feature, exceptional interface bonding between flax and PHU was confirmed by atomic force microscopy and rationalized by atomistic simulation. Without any treatment, an increase of 30% of stiffness and strength was unveiled compared to an epoxy benchmark, reaching 35 GPa and 440 MPa respectively. Related to the thermoreversible nature of hydroxyurethane moieties, cured flax-PHU were successfully self-welded and displayed promising properties, together with recyclability features. This opens advanced opportunities that cannot be reached with epoxy-based composites. Implementing CO 2 -derived thermosets in NFC could lead to more circular materials, critical for achieving sustainability goals.

Résumé : Context Primary hyperparathyroidism (PHPT) is commonly diagnosed in the setting of hypercalcemia, whereas normocalcemic primary hyperparathyroidism (NHPT) may be misdiagnosed. Objective Our objective was to compare hypercalcemic hyperparathyroidism (HPHPT) versus NHPT hypercalciuric renal stone patients. Design and Setting We took advantage of a routine calcium load test performed in hypercalciuric renal stone patients to assess retrospectively among PHPT patients, prevalence and characteristics of NHPT and HPHPT under a calcium restricted diet Results Among 1671 hypercalciuric patients included, 91 patients have a final diagnosis of PHPT(post load ionized calcium (iCa)>1.31 mmol/L and PTH>30 pg/ml). Prevalence of NHPT is 40% of all PHPT, however according to total serum calcium 4/35 NHPT and 7/56 HPHPT would have been misclassified in the other group. 18/35 NHPT and 40/56 HPHPT underwent parathyroidectomy. No significant characteristics related to parathyroid weight, stone composition or bone remodeling biomarkers is detected between groups. Whereas iCa is higher in HPHPT in fasting state and after calcium load, we found no difference for calcium diet, 24-hour calciuria, or calcitriol. Of notice, renal calcium excretion (FECa) post load increases by 303% in NHPT but only 176% in HPHPT (p=0.01) likely explained by a lesser PTH decrease (p=0.02). However, a strong negative association (p<0.0001) detected between pooled pre and post load iCa and PTH only in NHPT group suggests a persistent efficient PTH-CaSR control within parathyroid glands in this group. Conclusion Our data show the relevance of dynamic tests to unmask NHPT in hypercalciuric renal stone patients.

Résumé : Purpose: Amyotrophic lateral sclerosis (ALS) clinical variability, along with the lack of conclusive diagnostic instruments, result in average diagnosis delays of 9 months. This study aimed to assess whether metabolomic profiling of basal tears in ALS patients could act as a biological marker for diagnosing ALS, predicting prognosis, and discriminating between endophenotypes. Methods: A single-center prospective case-control study was conducted in France from September 2021 to March 2023 including patients with ALS according to the revised EI Escorial criteria. Two microliters of basal tears were collected using microcapillary glass tubes and analyzed with ultra-high performance liquid chromatography coupled with mass spectrometry. Both univariate and multivariate analyses were performed. Results: Twenty-five patients with ALS and 30 controls were included. No significant differences in metabolite levels were found between ALS and control groups (p > 0.05). The basal tear metabolome significantly discriminated bulbar and spinal forms of ALS based on 6 metabolites, among which 5 were decreased (aniline, trigonelline, caffeine, theophylline and methyl beta-D-galactoside) in the bulbar form and 1 was decreased in the spinal form (dodecanedioic acid). Conclusion: This study represents the first prospective analysis of basal tear metabolomics in individuals with ALS. Despite the inability to distinguish between ALS patients and controls based on metabolic signatures, these findings could contribute to understanding the phenotypic diversity of ALS. Notably, distinct metabolic profiles were identified that differentiate between the bulbar and spinal forms of the disease.

Résumé : The carbon sink in pantropical biomes play a crucial role in modulating the inter-annual variations of global terrestrial carbon balance and is threatened by extreme climate events. However, it has not been carefully examined whether an increase in tropical gross primary productivity (GPP) can compensate the decrease during precipitation anomalies. Using the asymmetry index (AI) and multiple GPP products, we assessed responses of pantropical GPP to precipitation anomalies during 2001-2022. Positive AI indicates that GPP increases are greater than GPP decreases during precipitation anomalies, and vice versa. Our results showed an average negative pantropical GPP asymmetry, that is, GPP decreases exceeded the GPP increases during precipitation anomalies. In addition, a positive AI was found in tropical hyper-arid and arid regions, which is opposite to the negative AI observed in tropical semi-arid, sub-humid, and humid regions. This suggest that tropical GPP asymmetry changes from positive to negative as the moisture increases. Notably, a significant decreasing trend of negative AI was observed over the entire tropical region, indicating that the negative effect of inter-annual precipitation variations on pantropical vegetation productivity has enhanced. Considering the model predicted increasing climate variability and extremes, the negative impact of precipitation variability on tropical carbon cycle may continue to intensify. Lastly, the divergence in AI estimates among multiple GPP products highlight the need to further improve our understanding of the response of tropical carbon cycle to climate changes, especially for the tropical humid regions.
Plain Language Summary Tropical biomes play an essential role in controlling the global carbon balance but has been threatened by extreme climate events recently. Whether the increase in tropical GPP can compensate the GPP decrease during precipitation anomalies is still poorly known. In this study, we found an average negative asymmetric GPP response to precipitation anomalies over the entire tropics during 2001-2022, that is, GPP decreases exceeded the GPP increases during precipitation anomalies. Meanwhile, we found that the tropical GPP was shifted from positive asymmetry to negative asymmetry as the moisture increases. Besides, a significantly decreasing trend of GPP asymmetry was observed over the study period, suggesting that the negative effect of inter-annual precipitation variations on pantropical GPP has intensified.
