Résumé : Background: Cryopyrin-associated periodic syndrome (CAPS) is associated with NLRP3 pathogenic variants, mostly located in the NACHT domain. Cold induced urticarial rash is one of the main clinical features. We however identified a series of 14 patients with pathogenic variants of the Y861 residue (p.Tyr861) of the LRR domain of NLRP3 and minimal prevalence of cold-induced urticarial rash. Objective: We aimed to address a possible genotype / phenotype correlation for CAPS patients and to investigate at the cellular levels the impact of the Y861C substitution (p.Tyr861Cys) on NLRP3 activation. Methods: Clinical features of 14 CAPS patients with heterozygous substitution at position 861 in the LRR domain of NLRP3 were compared to clinical features of 48 CAPS patients with pathogenic variants outside the LRR domain of NLRP3. IL-1β secretion by PBMCs and purified monocytes from patients and healthy donors was evaluated following LPS and MSU stimulation. Results: Patients with substitution at position 861 of NLRP3 demonstrated a higher prevalence of sensorineural hearing loss while being less prone to skin urticarial. In contrast to classical CAPS patients, cells from patients with a pathogenic variant at position 861 required an activation signal to secrete IL-1β but produced more IL-1β during the early and late phase of secretion than cells from healthy donors. Conclusion: Pathogenic variants of Y861 of NLRP3 drive a boost-dependent over secretion of IL-1β associated with an atypical CAPS phenotype.

Résumé : Alterations of the microbiome are linked to increasingly common diseases such as obesity, allergy, and inflammatory bowel disease. Post-industrial lifestyles are thought to contribute to the gut microbiome alterations that cause or aggravate these diseases. Comparing communities across the industrialization spectrum can reveal associations between gut microbiome alterations and lifestyle and health, and help pinpoint which specific aspect of the post-industrial lifestyle is linked to microbiome alterations. Here, we compare the gut microbiomes of 60 mother and infant pairs from rural and urban areas of Senegal over two time points. We find that urban mothers, who were more frequently overweight, had different gut microbiome compositions than rural mothers, showing an expansion of Lachnospiraceae and Enterobacter. Urban infants, on the other hand, showed a delayed gut microbiome maturation and a higher susceptibility to infectious diseases. Thus, we identify new microbiome features associated with industrialization, whose association with disease may be further investigated.

Résumé : Ce premier numéro des Cahiers Histoire de l’ASN est consacré au thème des « accidents nucléaires ». Si certains accidents nucléaires sont connus, au point d’ailleurs que leur nom propre est entré dans le langage courant, d’autres oscillent entre mémoire et oubli. C’est le cas pour deux accidents décrits dans ce numéro, ceux de Saint-Laurent-des-Eaux. Or, nous avons un devoir de construire une mémoire collective en capacité de servir pour demain. Trois idées clés nous semblent devoir être retenues à ce stade. D’abord, dans une société qui se veut sans risque, il est nécessaire de rappeler que le risque zéro n’existe pas et le nucléaire n’échappe pas à cette règle universelle. Comme le rappelle André-Claude Lacoste, président de l’ASN de 2006 à 2012, « personne ne peut garantir qu’il n’y aura jamais un accident grave en France. Il convient de faire deux choses : essayer de réduire la probabilité que cela arrive, ainsi que les conséquences, si cela arrive. C’est toute la philosophie de la sûreté nucléaire ». Ensuite, vis-à-vis des accidents du passé, il est nécessaire d’aller au-delà du caractère unique de ceux-ci pour en explorer les causes profondes et en tirer les enseignements qui permettront d’anticiper le potentiel accident, de le gérer au mieux, ainsi que la phase postaccidentelle. Nombre d’évolutions en matière organisationnelle ou de doctrine de sûreté nucléaire et de radioprotection proviennent de ce travail de retour d’expérience. Nous avons choisi de décrire ce travail au travers de cinq événements marquants, qui ont chacun généré des avancées majeures en matière de sûreté nucléaire et de radioprotection. Enfin, une des conséquences des accidents majeurs est bien l’émergence d’une conscience internationale des risques liés au nucléaire. Le message selon lequel la sûreté nucléaire est un bien commun et ne doit pas faire l’objet d’une compétition ou bien encore de manipulations géostratégiques, demeure, au regard des événements récents, plus que jamais d’actualité.

Résumé : Background: Obstructive coronary artery disease is the main cause of death worldwide. By tracking events and gaining feedback on patient management, the most relevant information is provided to public health services to further improve prognosis.Aims: To create an inclusive and accurate registry of all percutaneous coronary intervention (PCI) proce-dures performed in France, to assess and improve the quality of care and create research incentives. Also, to describe the methodology of this French national registry of interventional cardiology, and present early key findings. Methods: The France PCI registry is a multicentre observational registry that includes consecutive patients undergoing coronary angiography and/or PCI. The registry was set up to provide online data analysis and structured reports of PCI activity, including process of care measures and assessment of risk-adjusted outcomes in all French PCI centres that are willing to participate. More than 150 baseline data items, describing demographic status, PCI indications and techniques, and in-hospital and 1-year outcomes, are captured into local reporting software by medical doctors and local research technicians, with subsequent encryption and internet transfer to central data servers. Annual activity reports and scoring tools available on the France PCI website enable users to benchmark and improve clinical practices. External validation and consistency assessments are performed, with feedback of data completeness to centres.Results: Between 01 January 2014 and 31 December 2022, participating centres increased from six to 47, and collected 364,770 invasive coronary angiograms and 176,030 PCIs, including 54,049 non-ST -segment elevation myocardial infarction cases and 31,631 ST-segment elevation myocardial infarction cases. Fifteen studies stemming from the France PCI registry have already been published. Conclusions: This fully electronic, daily updated, high-quality, low-cost, national registry is sustainable, and is now expanding. Merging with medicoeconomic databases and nested randomized scientific studies are ongoing steps to expand its scientific potential.

Résumé : Objectives: Urinary tract infections (UTIs) are primarily caused by uropathogenic Escherichia coli (UPEC). This study aims to elucidate the role of the virulence factor HlyF in the epidemiology and pathophysiology of UTIs and investigate the dissemination of plasmids carrying the hlyF gene.Methods: An epidemiological analysis was conducted on a representative collection of 225 UPEC strains isolated from community-acquired infections. Selected hlyF+ strains were fully sequenced using a combination of Illumina and Nanopore technologies. To investigate the impact of HlyF, a murine model of UTI was utilized to compare clinical signs, bacterial loads in the bladder, kidney, and spleen, onset of bacteraemia, and inflammation through cytokine quantification among wild-type hlyF+ strains, isogenic mutants, and complemented mutants.Results: Our findings demonstrate that 20% of UPEC encode the HlyF protein. These hlyF+ UPEC strains exhibited enhanced virulence, frequently leading to pyelonephritis accompanied by bloodstream infections. Unlike typical UPEC strains, hlyF+ UPEC strains demonstrate a broader phylogroup distribution and possess a unique array of virulence factors and antimicrobial resistance genes, primarily carried by ColV-like plasmids. In the murine UTI model, expression of HlyF was linked to the UPECs' capacity to induce urosepsis and elicit an exacerbated inflammatory response, setting them apart from typical UPEC strains.Discussion: Overall, our results strongly support the notion that HlyF serves as a significant virulence factor for UPECs, and the dissemination of ColV-like plasmids encoding HlyF warrants further investigation. Résumé : We investigate a system composed of an ensemble of room-temperature rare-earth ions embedded in a bulk crystal, intrinsically coupled to internal strain via their sensitivity to the surrounding crystal field. We evidence the generation of a mechanical response under resonant atomic excitation. We find this motion to be the sum of two fundamental, resonant optomechanical backaction processes: a conservative, piezo-orbital mechanism, resulting from the modification of the crystal field associated with the promotion of the ions to their excited state, and a dissipative, nonradiative photothermal process related to the phonons generated throughout the atomic population relaxation. Our work expands the horizons of research in hybrid optomechanics, and unveils unexplored interactions that may be key for understanding the dephasing dynamics of ultracoherent rare-earth ions.

Résumé : In order to prevent damage to painted wooden panels from the cultural heritage sector, curators and conservators must provide the hygroscopic and thermal conditions necessary for their conservation. Furthermore, the design of physical reinforcements such as frames or cradles has to be considered. In this paper, experiments were designed to determine the kinematic response of a painting to hygrometric variations both before and after remedial conservation treatments were undertaken. This study was carried out in the Fabre Museum, Montpellier on the panel painting "La Sainte Trinité couronnant la Vierge" (« The Coronation of the Virgin by the Holy Trinity »), which was placed in a climate controlled showcase in a museum exhibition space. A glass panel on either side allowed visitors to see both sides of the panel during the experiment. At the same time, two stereoscopic devices allowed us to follow the relief, the shape deformation and strain of the panel on its front and back sides. First, the mechanical effect of the frame and cradle was studied. The majority of the deformation was found to be due to the cradle; initially the panel had a complex warp (-5 to 10 mm), decreasing to (-1 to +2 mm) when the cradle was removed. The continuous monitoring (two images per hour) was carrried out to assess the behaviour of the panel during imposed relative humidity (RH) variations of about 13% RH (between 65% RH for the maximum and 52% RH for the minimum) for both the absorption and desorption phases. The global mechanical effect on the curvature was very clear with an opposite direction of deflexion between absorption and desorption phases. A comparison of the behaviour of the panel with and without the cradle showed that the change in curvature was about twice as great (4 mm) without the cradle, and its direction moved from an initial slight negative curvature state to a positive one. In terms of strain on both the surfaces, we observed a horizontal shrinkage of about-0.1% during the desorption phase and a swelling of about +0.1% during the absorption phase for both the front and back sides of the panel. A further local study allowed us to assess the mechanical phenomenon for each board with the local determination of cupping angles and strains. This local study highlighted the change in behaviour of the panel without the cradle. On both sides, the local strains were decomposed into the mean strain and the deviation from the mean strain. The study highlighted the fact that the intensity of the mean strain (local swelling or shrinkage due to the hygroscopic changes) was the same for each configuration and was about 0.1%.
