Résumé : Background: After the Food and Drug Administration alert about antiepileptic medication and suicide, incident epilepsy has been associated with first or recurrent suicide attempts independently of psychiatric comorbidities and antiepileptic treatment. Following this thread, the aim of this study was to analyze if epilepsy was associated with a higher severity of lifetime suicide attempts (SAs).Methods: Analyses were carried out on 1677 adults hospitalized between 1999 and 2012 after a SA in a specialized ward for affective episodes. Five severity features were studied: frequent SAs (>2), early onset of first SA (≤26 years), history of violent SA, high suicide intent and high lethality of the SA. Adjusted logistic regression models were used to estimate the association between the lifetime diagnosis of epilepsy and the severity features.Results: Among suicide attempters, ninety-three patients reported a lifetime diagnosis of epilepsy (5.5%). Epileptic patients diagnosed after the first SA were more likely to be frequent suicide attempters than non-epileptic ones. They showed also higher SA planification scores.Limitations: Diagnosis accuracy is limited by the use of self-reports for epilepsy. The lack of precise information about the disease course and treatment have not allowed for further statistical analysis. With regard to psychiatric comorbidities, personality disorders could not be taken into account.Conclusions: Suicide attempters with epilepsy present an increased severity in some aspects of their suicidal behavior regardless of demographic and clinical variables. Our results give support to the existence of a bidirectional association between epilepsy and suicidal behavior.

Résumé : Background: Sodium channel blocker (SCB) infusion is used to unmask the electrocardiographic pattern of Brugada syndrome. The test may also induce premature ventricular complexes (PVCs) in individuals without Brugada pattern, the clinical relevance of which is little known.Objective: The purpose of this study was to describe the prevalence of short-coupled (Sc) PVCs induced by ajmaline or flecainide in patients with suspected or documented severe ventricular arrhythmias.Methods: We reviewed the SCB tests performed in 335 patients with suspected ventricular arrhythmias and structurally normal hearts in 9 centers. ScPVCs were defined as frequent and repetitive PVCs with an R-on-T pattern on SCB tests. Repeated SCB tests were performed in 7 patients and electrophysiological mapping of ScPVCs in 9.Results: Sixteen patients (8 men; mean age 36 ± 11 years) showed ScPVCs and were included. ScPVCs appeared 229 ± 118 seconds after the initiation of infusion and displayed coupling intervals of 288 ± 28 ms. ScPVC patterns were monomorphic in 12 patients, originating from the Purkinje system in mapped patients. Repetitive PVCs were induced in 15 patients (94%) including polymorphic ventricular tachycardias in 9 (56%). SCB infusion was repeated 45 (interquartile range 0.6-46) months later and induced identical ScPVC in all. SCB test was the only mean to reveal the malignant arrhythmia in 6 patients. Catheter ablation was performed in 9 patients, resulting in arrhythmia elimination in 8 with a follow-up of 6 (interquartile range 2-9) years.Conclusion: SCB can induce ScPVC, mostly from Purkinje tissue, in a small subset of patients with idiopathic ventricular arrhythmias. Its high reproducibility suggests a distinct individual mechanism.

Résumé : Purpose: The decision for subthalamic deep brain stimulation (STN-DBS) in Parkinson's disease (PD) relies on clinical predictors. Whether genetic variables could predict favourable or unfavourable decisions is under investigation. Objective: First, we aimed to reproduce the previous observation that SNCA rs356220 was associated with favourable STN-DBS motor response. In additional exploratory analyses, we studied if other PD risk and progression variants from the latest GWAS are associated with therapeutic outcome. Further, we evaluated the predictive value of polygenic risk scores. Methods: We comprehensively genotyped patients from the EarlyStim cohort using NeuroChip, and assessed the clinico-genetic associations with longitudinal outcome parameters. Results: The SNCA rs356220 variant did not predict UPDRS III outcomes. However, it was associated with quality of life improvement in secondary analyses. Several polymorphisms from previously identified GWAS hits predicted motor or quality of life outcomes in DBS patients. Polygenic risk scores did not predict any outcome parameter. Conclusions: Our findings support the hypothesis that different common genetic markers are associated with favourable quality of life outcomes of STN-DBS in PD. These findings can be the basis for further validation in larger and independent cohorts.

Résumé : Background In most low-to-middle-income countries, HIV control at the population level among people who inject drugs (PWID) remains a major challenge. We aimed to demonstrate that an innovative intervention can identify HIV-positive PWID in the community who are not treated efficiently, and get them treated efficiently. Methods Between 2016 and 2020, we implemented an intervention consisting of mass HIV screening of PWID using three annual respondent-driven sampling surveys (RDSS) and a post-intervention evaluation RDSS in community-based organisation (CBO) sites, coupled with peer support to facilitate/improve access to antiretroviral and methadone therapy in Haiphong, Vietnam. The primary outcome was the proportion of identified uncontrolled HIV-positive PWID who achieved viral control. We also estimated the potential effect of the intervention on the proportion of PWID with HIV RNA >1000 copies/mL among all PWID during the study period. Findings Over the three RDSS, 3150 different PWID were screened, i.e. two-thirds of the estimated population size. They all injected heroin, their median age was of 39 years, 95% were male, 26.5% were HIV-infected, and 78.6% of the latter had HIV RNA ≤1000 copies/mL. Among the 177 PWID identified with an unsuppressed viral load, 73 (41.2%) achieved viral suppression at the final visit. HIV viremia decreased from 7.2% at baseline to 2.9% at the final RDSS (p<0.001). Up to 42% of this observed reduction may be explained by the intervention, in the absence of any external intervention targeting PWID during the study period. Interpretation Mass community-based screening using RDSS coupled with CBO support is a powerful tool to rapidly identify untreated HIV-positive PWID and (re)link them to care.

Résumé : The Aiguilles-Rouges and Mont-Blanc massifs represent a segment of a crustal-scale transpressional shear zone named the East Variscan Shear Zone (EVSZ) along which two plutonic pulses occurred during Early and Late Carboniferous times. The aim of this study is to constrain the relationships between the dynamics of the crustal-scale shear zone, the mechanisms of pluton emplacement at different structural levels within the crust and the magma sources. A detailed structural analysis of the whole massif highlights the crustal-scale anastomosed network of the EVSZ. Microstructural observations and LA-ICPMS U–Th–Pb zircon ages from large plutons constrain the beginning of the transpression at ca. 340 Ma. From 340 to 305 Ma, the EVSZ broadened and formed a 25 km-wide dextral S–C–C’ anastomosed shear zone network with dilation zones acting as preferential pathways for melt migration and pluton growths. Moreover, LA-ICPMS U–Th–Pb zircon ages from small magmatic bodies (i.e. pegmatite, aplite and microgranite) indicate an Ordovician-age inheritance component. Field evidences and zircon inheritance indicate that Late Carboniferous granitic melts are mainly derived from water-fluxed melting of Ordovician orthogneiss with the input of mantle-source derived magmas. Over time, the growth of the dextral anastomosed network enhanced water transfer through the shear zones and water-fluxed melting to produce more anatectic melts.

Résumé : This paper presents significant improvements to the nominal control architecture of the open-access Solo-12 quadruped that were done to implement and compare different centroidal Model Predictive Controllers (MPC). This work was motivated by our previous study in which various MPC schemes of increasing complexity were tested in simulation. They range from a simplified linearized model with contact points fixed by a heuristic, to a nonlinear one that also optimizes the contact points locations. We describe the developments that were necessary to implement such control schemes on the real robot while doubling its maximum velocity. Notably, to synthesize a stable whole-body controller, the inverse dynamics resolution was replaced by a mixed inverse kinematics and quadratic programming scheme. These developments enabled the successful deployments of the various centroidal MPCs on Solo-12. Experimental results show that all MPCs lead to quite similar performances with the proposed whole-body controller and, as a consequence, do not confirm the result of previous simulation study that concluded on the preeminence of the nonlinear centroidal MPC optimizing both the center of mass trajectory and the foot placements.

Résumé : Background: Point-Of-Care (POC) diagnosis of life-threatening community-acquired meningitis currently relies on multiplexed RT-PCR assays, that lack genotyping and antibiotic susceptibility profiling. We assessed the usefulness of real-time metagenomics (RTM) directly applied to the cerebrospinal fluid (CSF) for the identification, typing and susceptibility profiling of pathogens responsible for community-acquired meningitis. Methods: A series of 52 CSF samples from patients suspected of having community-acquired meningitis, were investigated at POC by direct RTM in parallel to routine real-time multiplex PCR (RT-PCR) and bacterial culture, for the detection of pathogens. RTM-generated sequences were blasted in real-time against an in-house database incorporating the panel of 12 most prevalent pathogens and against NCBI using EPI2ME online software, for pathogen identification. In-silico antibiogram and genotype prediction were determined using the ResFinder bio-tool and MLST online software. Findings: Over eight months, routine multiplex RT-PCR yielded 49/52 positive CSFs, including 21 Streptococcus pneumoniae, nine Neisseria meningitidis, eight Haemophilus influenzae, three Streptococcus agalactiae, three Herpesvirus-1, two Listeria monocytogenes, and one each of Escherichia coli, Staphylococcus aureus and Varicella-Zoster Virus. Parallel RTM agreed with the results of 47/52 CSFs and revealed two discordant multiplex RT-PCR false positives, one H. influenzae and one S. pneumoniae. Both multiplex RT-PCR and RTM agreed on the negativity of three CSFs. While multiplex RT-PCR routinely took 90 min, RTM took 120 min, although the pipeline analysis detected the pathogen genome after 20 min of sequencing in 33 CSF samples; and after two hours in 14 additional CSFs; yielding> 50% genome coverage in 19 CSFs. RTM identified 14 pathogen genotypes, including a majority of H. influenzae b, N. meningitidis B and S. pneumoniae 11A and 3A. In all 16 susceptible cultured bacteria, the in-silico antibiogram agreed with the in-vitro antibiogram in 10 cases, available within 48 h in routine bacteriology. Interpretation: In addition to pathogen detection, RTM applied to CSF samples offered supplementary information on bacterial profiling and genotyping. These data provide the proof-of-concept that RTM could be implemented in a POC laboratory for one-shot diagnostic and genomic surveillance of pathogens responsible for life-threatening meningitis.

Résumé : Nanometer size p-n heterojunction has been created from CuO and ZnO anisotropic nanoparticles prepared by a one-pot organometallic approach. The method is based on the hydrolysis or oxidation of an adequate metal-organic precursors in pure octylamine. The CuO and ZnO nanostructures were dispersed in ethanol and then, mixed at different mass ratios, i.e. CuO(75%)/ZnO(25%), CuO(50%)/ZnO(50%) and CuO(25%)/ZnO(75%). Finally, the CuO and ZnO suspensions and their mixtures were deposited on miniaturized gas sensors substrates by an ink-jet printing method and heated up gradually to 550 oC in ambient air. Then, the as-prepared sensors have been exposed to CO (100 ppm), C3H8 (100 ppm) and NH3 (5 ppm) at different working temperatures (from 75 oC to 400 oC) and under 50 % of relative humidity (RH). Among all prepared sensors the one based on the mixture of CuO (75 %) and ZnO (25 %) presents a very sensitive and selective response to CO. Indeed, at the operating temperature of 165 oC, a high sensitivity towards CO was obtained (SCO=624 %). In these conditions, the sensor exhibited low sensitivity to other tested gases (KCO/C3H8= 14.5 and KCO/NH3= 26) but its response time was quite long (t90=2.3 min) and the recovery was very sluggish (t10>20 min). Therefore, it was better to increase the working temperature up to 300 oC. Although the sensitivity and selectivity towards CO worsened (SCO=177%, KCO/C3H8= 4.7 and KCO/NH3= 9.8) the response/recovery time decreased to 50 s/4.5 min. The gas sensing performances of the CuO(75%)/ZnO(25%) composite was attributed to both high surface to volume ratio of the prepared nanostructures and the p-n heterojunction established between the CuO and ZnO nanoparticles.
