Résumé : Aims In arrhythmogenic cardiomyopathy (ACM), sustained ventricular tachycardia (VT) typically displays a left bundle branch block (LBBB) morphology while a right bundle branch block (RBBB) morphology is rare. The present study assesses the VT morphology in ACM patients with sustained VT and their clinical and genetic characteristics. Methods and results Twenty-six centres from 11 European countries provided information on 954 ACM patients who had >= 1 episode of sustained VT spontaneously documented during patients' clinical course. Arrhythmogenic cardiomyopathy was defined according to the 2010 Task Force Criteria, and VT morphology according to the QRS pattern in V1. Overall, 882 (92.5%) patients displayed LBBB-VT alone and 72 (7.5%) RBBB-VT [alone in 42 (4.4%) or in combination with LBBB-VT in 30 (3.1%)]. Male sex prevalence was 79.3%, 88.1%, and 56.7% in the LBBB-VT, RBBB-VT, and LBBB + RBBB-VT groups, respectively (P = 0.007). First RBBB-VT occurred 5 years after the first LBBB-VT (46.5 +/- 14.4 vs 41.1 +/- 15.8 years, P = 0.011). An implanted cardioverter-defibrillator was more frequently implanted in the RBBB-VT (92.9%) and the LBBB + RBBB-VT groups (90%) than in the LBBB-VT group (68.1%) (P < 0.001). Mutations in PKP2 predominated in the LBBB-VT (65.2%) and the LBBB + RBBB-VT (41.7%) groups while DSP mutations predominated in the RBBB-VT group (45.5%). By multivariable analysis, female sex was associated with LBBB + RBBB-VT (P = 0.011) while DSP mutations were associated with RBBB-VT (P < 0.001). After a median follow-up of 103 (51-185) months, death occurred in 106 (11.1%) patients with no intergroup difference (P = 0.176). Conclusion RBBB-VT accounts for a significant proportion of sustained VTs in ACM. Sex and type of pathogenic mutations were associated with VT type, female sex with LBBB + RBBB-VT, and DSP mutation with RBBB-VT.

Résumé : Introduction Status epilepticus (SE) is a common life-threatening neurological emergency that can cause long-term impairments. Overall outcomes remain poor. Major efforts are required to clarify the epidemiology of SE and the determinants of outcomes, thereby identifying targets for improved management. Methods and analysis ICTAL Registry is a multicentre open cohort of critically ill patients with convulsive, non-convulsive or psychogenic non-epileptic SE. Observational methods are applied to collect uniform data. The goal of the ICTAL Registry is to collect high-quality information on a large number of patients, thereby allowing elucidation of the pathophysiological mechanisms involved in mortality and morbidity. The registry structure is modular, with a large core data set and the opportunity for research teams to create satellite data sets for observational or interventional studies (eg, cohort multiple randomised controlled trials, cross-sectional studies and short-term and long-term longitudinal outcome studies). The availability of core data will hasten patient recruitment to studies, while also decreasing costs. Importantly, the vast amount of data from a large number of patients will allow valid subgroup analyses, which are expected to identify patient populations requiring specific treatment strategies. The results of the studies will have a broad spectrum of application, particularly given the multidisciplinary approach used by the IctalGroup research network. Ethics and dissemination The ICTAL Registry protocol was approved by the ethics committee of the French Intensive Care Society (#CE_SRLF 19-68 and 19-68a). Patients or their relatives/proxies received written information to the use of the retrospectively collected and pseudonymised data, in compliance with French law. Prospectively included patients receive written consent form as soon as they recover decision-making competency; if they refuse consent, they are excluded from the registry. Data from the registry will be disseminated via conference presentations and peer-reviewed publications. Trial registration number NCT03457831.

Résumé : Ce rapport rend compte des travaux réalisés au cours de l’année 2021, dans le cadre du projet de recherche relatif à la Butte de César, située sur l’oppidum des Châtelliers à Amboise (37). Après cinq années consacrées aux recherches de terrain (2015-2019), cette nouvelle campagne a permis de se pencher plus spécifiquement sur le traitement du mobilier. Les quantités importantes d’objets mis au jour lors des fouilles nécessitaient en effet une reprise et une homogénéisation des inventaires. En outre, plusieurs problématiques soulevées lors des précédents rapports exigeaient que des analyses complémentaires soient entreprises. Au cours des derniers mois, une grande partie de ces objectifs ont été remplis, permettant ainsi d’affiner la caractérisation des quatre grandes périodes d’occupation mises en évidence dans l’emprise de la fouille. S’agissant de la première de ces périodes (Ier âge du Fer), les analyses menées cette année ont permis de confirmer une grande partie des hypothèses relatives à l’architecture et à la chronologie du tumulus princier, formulées lors de la dernière campagne. Une analyse micro-morphologique a permis de documenter les grandes étapes de l’édification du tertre, tandis que des analyses isotopiques et anthracologiques ont permis de mieux caractériser les matériaux mis en œuvre dans la palissade périphérique. Une partie importante des travaux ont concerné la seconde période d’occupation (-60 av. n.è. / 20), qui est aussi la mieux représentée en nombre de structure et en quantité de mobilier. Les inventaires de la céramique, de l’instrumentum et des monnaies sont désormais homogénéisés, livrant ainsi une vision exhaustive des faciès de consommation dans ce quartier de l’oppidum. Les problématiques soulevées successivement au cours de la fouille – mise en phase, pratiques artisanales et rituelles, faciès romanisés - peuvent ainsi être abordées sur des bases fiables. Par la quantité du mobilier mis au jour, et par le caractère inédit de plusieurs des découvertes relatives à cette période, il est désormais confirmé que le quartier de la Butte de César se révèle très original au sein de l’oppidum. L’apport des études archéozoologique et anthropologique a également été décisif cette année, avec la mise en évidence de pratiques spécifiques. S’agissant de la troisième période (Ier et IIe s. de n.è.) et de la quatrième (XVe – XVIIIe s.), les travaux réalisés cette année ont permis d’achever le traitement de leurs mobiliers respectifs. La caractérisation de ces occupations a pu être affiner. La chronologie du point d’artillerie de la Renaissance a notamment pu être améliorée grâce à l’étude du mobilier céramique, ainsi que de nouvelles datations radiocarbones.

Résumé : Strict priority policies in a stochastic system with abandonment In the technical note “Stochastic scheduling with abandonment: Necessary and sufficient conditions for the optimality of a strict priority policy,” Chen, Gayon, and Lemaire consider a stochastic scheduling problem in which jobs abandon when their waiting time exceeds their lifetime. Such a problem arises, for example, in call centers or emergency systems. It is known that the optimal policy is a strict priority policy under some sets of conditions. The authors provide the first set of necessary and sufficient conditions for a problem with two types of jobs. They also provide conjectures to guide toward generalizations of the proposed conditions.

Résumé : Background: Spinal cord injury often results in erectile dysfunction and an ejaculation along with impaired semen parameters. Fertility is a major concern in spinal cord injury adult males and some fear that the delay post-spinal cord injury may negatively affect sperm quality. Objectives: We aimed to (i) assess semen parameters over time in SCI patients according to age at spinal cord injury, time post-spinal cord injury, and the spinal cord injury level and completeness and (ii) measure markers in semen for inflammation and marker of oxidative stress to investigate their impact on sperm parameters. Materials and Methods: The study is a prospective, longitudinal, pilot study over 18 months. Thirty-five men with spinal cord injury from 18 to 60 years of age were enrolled. Their mean age was 29.4 ± 6.4 years. Semen retrieval was scheduled every 6 months, allowing analysis of four ejaculates, in association with measurement of granulocyte and seminal plasma elastase concentrations to assess markers in semen for inflammation and spermatozoa DNA fragmentation to assess oxidative stress. Results: Based on reference limits, a normal total sperm number, decreased motility and vitality of the spermatozoa, and increased morphological abnormalities were found. Mean round cell and granulocyte concentrations were elevated in the semen. Markers in semen for inflammation and marker of oxidative stress were elevated in several semen samples, compared to reference limits. However, neither the presence of markers in semen for inflammation or oxidative stress, the completeness or the level of the spinal cord lesion, the age or the time post-spinal cord injury had a negative impact on the semen quality over time. Discussion: There was no significant decline in semen quality in spinal cord injury patients over time within the limitations of this pilot study. Moreover, a chronic genital inflammatory status was not associated with impairment of semen quality. Conclusion: The present findings are reassuring for men with spinal cord injury and could guide the management of their reproductive ability. According to these preliminary data, not all spinal cord injury patients who are able to ejaculate require systematic freezing of their spermatozoa.

Résumé : Il est des relations entre cinéastes et compositeurs qui durent. D’autres ne sont qu’éphémères. Pourtant l’une et l’autre peuvent, à des degrés divers, marquer une carrière, servir de bornes. Très tôt entré dans le cercle de la Nouvelle Vague, Antoine Duhamel va composer pour Jean-Luc Godard, François Truffaut, Jean-Daniel Pollet… Jamais pour Claude Chabrol, dont il respecte la collaboration avec son ami Pierre Jansen. Si sa rencontre avec Godard fut de celles que l’on dira éphémères, elle n’en marque pas moins profondément l’ensemble de son œuvre pour le cinéma : plus que celle de Week-end, la musique de Pierrot le Fou apparaît bien souvent comme une référence absolue en la matière. Ses relations avec Truffaut furent tout autres. Duhamel avoue avoir été déçu par sa collaboration avec le réalisateur de la saga d’Antoine Doinel. « Pollet et Godard m’avaient entraîné très haut, Truffaut me faisait retomber. »Nous aurions pu choisir, parmi les nombreuses amitiés artistiques qui ont jalonné la carrière de Duhamel, d’évoquer Philippe Condroyer ou encore Bertrand Tavernier. C’est pourtant à celui qu’il rencontre à l’aube naissante d’une saga que nous allons nous intéresser. Cet autre, c’est Jean-Daniel Pollet.

Résumé : Understanding the critical soil moisture (SM) threshold (θcrit) of plant water stress and land surface energy partitioning is a basis to evaluate drought impacts and improve models for predicting future ecosystem condition and climate. Quantifying the θcrit across biomes and climates is challenging because observations of surface energy fluxes and SM remain sparse. Here, we used the latest database of eddy covariance measurements to estimate θcrit across Europe by evaluating evaporative fraction (EF)-SM relationships and investigating the covariance between vapor pressure deficit (VPD) and gross primary production (GPP) during SM dry-down periods. We found that the θcrit and soil matric potential threshold in Europe are 16.5% and −0.7 MPa, respectively. Surface energy partitioning characteristics varied among different vegetation types; EF in savannas had the highest sensitivities to SM in water-limited stage, and the lowest in forests. The sign of the covariance between daily VPD and GPP consistently changed from positive to negative during dry-down across all sites when EF shifted from relatively high to low values. This sign of the covariance changed after longer period of SM decline in forests than in grasslands and savannas. Estimated θcrit from the VPD–GPP covariance method match well with the EF–SM method, showing this covariance method can be used to detect the θcrit. We further found that soil texture dominates the spatial variability of θcrit while shortwave radiation and VPD are the major drivers in determining the spatial pattern of EF sensitivities. Our results highlight for the first time that the sign change of the covariance between daily VPD and GPP can be used as an indicator of how ecosystems transition from energy to SM limitation. We also characterized the corresponding θcrit and its drivers across diverse ecosystems in Europe, an essential variable to improve the representation of water stress in land surface models.

Résumé : Epitaxial clusters of chromium and chromium-vanadium oxides are studied by giant magneto-resistivity measurements, X-ray absorption spectrometry and circular magnetic circular dichroism. They turn out to carry a small magnetic moment that follows a superparamagnetic behavior.
