Résumé : Recent virus outbreaks have revealed a critical need for large scale serological assays. However, many available tests either require a cumbersome, costly apparatus or lack the availability of full automation. In order to address these limitations, we describe a homogeneous assay for antibody detection via measurement of superparamagnetic particles agglutination. Application of a magnetic field permits to overcome the limitations governed by Brownian translational diffusion in conventional assays and results in an important acceleration of the aggregation process as well as an improvement of the limit of detection. Furthermore, the use of protein-concentrated fluid such as 5 times-diluted human plasma does not impair the performances of the method. Screening of human plasma samples shows a strict discrimination between seropositive and seronegative samples in an assay duration as short as 14 s. The sensitivity of this method, combined with its quickness and simplicity, makes it a promising diagnostic tool.

Résumé : Background Endovascular therapy (EVT) is effective and safe in patients with tandem occlusion. The benefit of intravenous thrombolysis (IVT) prior to EVT in acute tandem occlusion is debatable. Objective To compare EVT alone with EVT plus IVT in patients with acute ischemic stroke due to anterior circulation tandem occlusions. Methods This is an individual patient pooled analysis of the Thrombectomy In TANdem lesions (TITAN) and Endovascular Treatment in Ischemic Stroke (ETIS) Registries. Patients were divided into two groups based on prior IVT treatment: (1) IVT+ group, which included patients who received IVT prior to EVT, (2) IVT− group, which included patients who did not receive IVT prior to EVT. Propensity score (inverse probability of treatment weighting (IPTW)) was used to reduce baseline between-group differences. The primary outcome was favorable outcome—that is, modified Rankin Scale (mRS) score 0 to 2 at 90 days. Results Overall, 602 consecutive patients with an acute stroke with tandem occlusion were included (380 and 222 in the bridging therapy and EVT alone groups, respectively). Onset to imaging time was shorter in the IVT+ group (median 103 vs 140 min). In contrast, imaging to puncture time was longer in the IVT+ group (median 107 vs 91 min). In IPTW analysis, the IVT+ group had higher odds of favorable outcome, excellent outcome (90-day mRS score 0–1), and successful reperfusion (modified Thrombolysis in Cerebral Infarction score 2b/3 at the end of EVT). There was no difference in the risk of significant hemorrhagic complications between groups. In secondary analysis of patients treated with acute cervical internal carotid artery stenting, bridging therapy was associated with higher odds of favorable outcome and lower odds of mortality at 90 days. Conclusions Our results suggest that bridging therapy in patients with acute ischemic stroke due to anterior tandem occlusion is safe and may improve functional outcome, even in the setting of acute cervical internal carotid artery stenting during EVT.

Résumé : Vegetation optical depth (VOD), as a microwave-based vegetation index for vegetation water and biomass content, is increasingly used to study the impact of global climate and environmental changes on vegetation. Currently, VOD is mainly retrieved from passive microwave data and few studies focused on VOD retrievals from active microwave data. The Advanced SCATterometer (ASCAT) provides long-term C-band backscatter data at Vertical-Vertical (VV) polarization. In this study, a new ASCAT INRAE Bordeaux (IB) VOD (hereafter, IB VOD), was developed based on the Water Cloud Model (WCM) coupled with the Ulaby linear model for soil backscattering. The main features of IB VOD are that (i) the ERA5-Land soil moisture (SM) dataset was used as an auxiliary SM dataset in the retrievals, (ii) pixel-based soil model parameters were mapped using Random Forest (RF), and (iii) the vegetation model parameter was calibrated for each day. The IB VOD product was retrieved over Africa during 2015–2019, and its performances were evaluated in space and time by comparing with aboveground biomass (AGB), lidar tree height (TH), normalized difference vegetation index (NDVI), enhanced vegetation index (EVI) and leaf area index (LAI). Results were inter-compared with three other VOD products at the same frequency. In terms of spatial correlation with AGB (R = 0.92) and TH (R = 0.89), IB VOD outperforms the other VOD products, suggesting IB VOD has a strong ability to capture spatial patterns of AGB and TH. By comparing all VOD products against NDVI, EVI and LAI, we found that the highest temporal correlation with NDVI (EVI, LAI) was obtained with IB VOD over 29.94% (36.65%, 30.19%) of the study region. Considering all three vegetations indices, highest temporal correlation values with IB VOD could be particularly noted for deciduous broadleaf forests, woody savannas and savannas.

Résumé : BACKGROUND and AIMS: Good outcomes after liver transplantation (LT) have been reported after successfully downstaging to Milan criteria in more advanced hepatocellular carcinoma (HCC). We aimed to compare post-LT outcomes in patients receiving locoregional therapies (LRT) before LT according to Milan criteria and University of California San Francisco downstaging (UCSF-DS) protocol and ’all-comers’. METHODS: This multicentre cohort study included patients who received any LRT before LT from Europe and Latin America (2000-2018). We excluded patients with alpha-foetoprotein (AFP) above 1,000 ng/ml. Competing risk regression analysis for HCC recurrence was conducted, estimating subdistribution hazard ratios (SHRs) and corresponding 95% CIs. RESULTS: From 2,441 LT patients, 70.1% received LRT before LT (n = 1,711). Of these, 80.6% were within Milan, 12.0% within UCSF-DS, and 7.4% all-comers. Successful downstaging was achieved in 45.2% (CI 34.8-55.8) and 38.2% (CI 25.4-52.3) of the UCSF-DS group and all-comers, respectively. The risk of recurrence was higher for all-comers (SHR 6.01 [p <0.0001]) and not significantly higher for the UCSF-DS group (SHR 1.60 [p = 0.32]), compared with patients remaining within Milan. The all-comers presented more frequent features of aggressive HCC and higher tumour burden at explant. Among the UCSF-DS group, an AFP value of ≤20 ng/ml at listing was associated with lower recurrence (SHR 2.01 [p = 0.006]) and better survival. However, recurrence was still significantly high irrespective of AFP ≤20 ng/ml in all-comers. CONCLUSIONS: Patients within the UCSF-DS protocol at listing have similar post-transplant outcomes compared with those within Milan when successfully downstaged. Meanwhile, all-comers have a higher recurrence and inferior survival irrespective of response to LRT. Additionally, in the UCSF-DS group, an ALP of ≤20 ng/ml might be a novel tool to optimise selection of candidates for LT. CLINICAL TRIAL NUMBER: This study was registered as part of an open public registry (NCT03775863). LAY SUMMARY: Patients with more extended HCC (within the UCSF-DS protocol) successfully downstaged to the conventional Milan criteria do not have a higher recurrence rate after LT compared with the group remaining in the Milan criteria from listing to transplantation. Moreover, in the UCSF-DS patient group, an ALP value equal to or below 20 ng/ml at listing might be a novel tool to further optimise selection of candidates for LT.

Résumé : In the recent past, we observed an increased risk of cancer in the HIV population due to the development of antiretroviral therapies that decreased mortality caused by HIV-specific infections. This particularly fragile population is frequently excluded from clinical trials, and up-to-date recommendations for these patients are lacking. Only few cases of HIV patients suffering from cancer and undergoing first-line immunotherapy have been reported so far. Here we report the largest known study of HIV patients with non-small cell lung cancer (NSCLC) (5 patients) undergoing first-line immunotherapy by pembrolizumab, following CANCERVIH group selection. Our results are consistent with previous case reports concerning safety of immunotherapy in HIV patients, showing no severe or fatal toxicity, opportunistic infections, or immune reconstitution inflammatory syndrome (IRIS). Moreover, pembrolizumab did not seem to modify HIV viral parameters. We also assessed effectiveness of immunotherapy in these HIV-immunosuppressed patients: the average survival was 9.8 months, with three patients showing rapid progression and two partial response. However, as well as safety and drug-to-drug interactions, the effectiveness of first-line immunotherapy in people living with HIV (PLWHIV) needs to be supported by larger studies.

Résumé : Some properties of chaotic dynamical systems can be probed through features of recurrences, also called analogs. In practice, analogs are nearest neighbours of the state of a system, taken from a large database called the catalog. Analogs have been used in many atmospheric applications including forecasts, downscaling, predictability estimation, and attribution of extreme events. The distances of the analogs to the target state condition the performances of analog applications. These distances can be viewed as random variables, and their probability distributions can be related to the catalog size and properties of the system at stake. A few studies have focused on the first moments of return time statistics for the best analog, fixing an objective of maximum distance from this analog to the target state. However, for practical use and to reduce estimation variance, applications usually require not just one, but many analogs. In this paper, we evaluate from a theoretical standpoint and with numerical experiments the probability distributions of the $K$-best analog-to-target distances. We show that dimensionality plays a role on the size of the catalog needed to find good analogs, and also on the relative means and variances of the $K$-best analogs. Our results are based on recently developed tools from dynamical systems theory. These findings are illustrated with numerical simulations of a well-known chaotic dynamical system and on 10m-wind reanalysis data in north-west France. A practical application of our derivations for the purpose of objective-based dimension reduction is shown using the same reanalysis data.
