Résumé : BACKGROUND: Mycophenolate mofetil (MMF) monotherapy following liver transplantation (LT) remains controversial due to a risk of acute rejection. The aim of this study was to report the largest multicenter experience of the use a MMF monotherapy guided by therapeutic drug monitoring using pharmacoslope modeling and Bayesian estimations of the MPA inter-dose AUC (BEMPAMETHODS: MMF daily doses were adjusted to reach the BEMPARESULTS: From 2000-2014, in 2 transplantation centers, 94 liver transplant recipients received MMF monotherapy 6.5±4 years after LT. The mean BEMPACONCLUSIONS: MMF monotherapy regimen appears usually safe and beneficial, with low risk of acute rejection and eGFR improvement. Therapeutic drug monitoring strategy seemed useful by identifying 14% of patients with low MMF exposure.

Résumé : Objective: Primary hypobetalipoproteinemia is characterized by LDL-C (low-density lipoprotein cholesterol) concentrations below the fifth percentile. Primary hypobetalipoproteinemia mostly results from heterozygous mutations in the APOB (apolipoprotein B) and PCSK9 genes, and a polygenic origin is hypothesized in the remaining cases. Hypobetalipoproteinemia patients present an increased risk of nonalcoholic fatty liver disease and steatohepatitis. Here, we compared hepatic alterations between monogenic, polygenic, and primary hypobetalipoproteinemia of unknown cause. Approach and Results: Targeted next-generation sequencing was performed in a cohort of 111 patients with hypobetalipoproteinemia to assess monogenic and polygenic origins using an LDL-C-dedicated polygenic risk score. Forty patients (36%) had monogenic hypobetalipoproteinemia, 38 (34%) had polygenic hypobetalipoproteinemia, and 33 subjects (30%) had hypobetalipoproteinemia from an unknown cause. Patients with monogenic hypobetalipoproteinemia had lower LDL-C and apolipoprotein B plasma levels compared with those with polygenic hypobetalipoproteinemia. Liver function was assessed by hepatic ultrasonography and liver enzymes levels. Fifty-nine percent of patients with primary hypobetalipoproteinemia presented with liver steatosis, whereas 21% had increased alanine aminotransferase suggestive of liver injury. Monogenic hypobetalipoproteinemia was also associated with an increased prevalence of liver steatosis (81% versus 29%, P<0.001) and liver injury (47% versus 0%) compared with polygenic hypobetalipoproteinemia.Conclusions: This study highlights the importance of genetic diagnosis in the clinical care of primary hypobetalipoproteinemia patients. It shows for the first time that a polygenic origin of hypobetalipoproteinemia is associated with a lower risk of liver steatosis and liver injury versus monogenic hypobetalipoproteinemia. Thus, polygenic risk score is a useful tool to establish a more personalized follow-up of primary hypobetalipoproteinemia patients.

Résumé : Importance Severe pneumonia with hyperinflammation and elevated interleukin-6 is a common presentation of coronavirus disease 2019 (COVID-19). Objective To determine whether tocilizumab (TCZ) improves outcomes of patients hospitalized with moderate-to-severe COVID-19 pneumonia. Design, Setting, and Particpants This cohort-embedded, investigator-initiated, multicenter, open-label, bayesian randomized clinical trial investigating patients with COVID-19 and moderate or severe pneumonia requiring at least 3 L/min of oxygen but without ventilation or admission to the intensive care unit was conducted between March 31, 2020, to April 18, 2020, with follow-up through 28 days. Patients were recruited from 9 university hospitals in France. Analyses were performed on an intention-to-treat basis with no correction for multiplicity for secondary outcomes. Interventions Patients were randomly assigned to receive TCZ, 8 mg/kg, intravenously plus usual care on day 1 and on day 3 if clinically indicated (TCZ group) or to receive usual care alone (UC group). Usual care included antibiotic agents, antiviral agents, corticosteroids, vasopressor support, and anticoagulants. Main Outcomes and Measures Primary outcomes were scores higher than 5 on the World Health Organization 10-point Clinical Progression Scale (WHO-CPS) on day 4 and survival without need of ventilation (including noninvasive ventilation) at day 14. Secondary outcomes were clinical status assessed with the WHO-CPS scores at day 7 and day 14, overall survival, time to discharge, time to oxygen supply independency, biological factors such as C-reactive protein level, and adverse events. Results Of 131 patients, 64 patients were randomly assigned to the TCZ group and 67 to UC group; 1 patient in the TCZ group withdrew consent and was not included in the analysis. Of the 130 patients, 42 were women (32%), and median (interquartile range) age was 64 (57.1-74.3) years. In the TCZ group, 12 patients had a WHO-CPS score greater than 5 at day 4 vs 19 in the UC group (median posterior absolute risk difference [ARD] −9.0%; 90% credible interval [CrI], −21.0 to 3.1), with a posterior probability of negative ARD of 89.0% not achieving the 95% predefined efficacy threshold. At day 14, 12% (95% CI −28% to 4%) fewer patients needed noninvasive ventilation (NIV) or mechanical ventilation (MV) or died in the TCZ group than in the UC group (24% vs 36%, median posterior hazard ratio [HR] 0.58; 90% CrI, 0.33-1.00), with a posterior probability of HR less than 1 of 95.0%, achieving the predefined efficacy threshold. The HR for MV or death was 0.58 (90% CrI, 0.30 to 1.09). At day 28, 7 patients had died in the TCZ group and 8 in the UC group (adjusted HR, 0.92; 95% CI 0.33-2.53). Serious adverse events occurred in 20 (32%) patients in the TCZ group and 29 (43%) in the UC group (P = .21). Conclusions and Relevance In this randomized clinical trial of patients with COVID-19 and pneumonia requiring oxygen support but not admitted to the intensive care unit, TCZ did not reduce WHO-CPS scores lower than 5 at day 4 but might have reduced the risk of NIV, MV, or death by day 14. No difference on day 28 mortality was found. Further studies are necessary for confirming these preliminary results. Trial Registration ClinicalTrials.gov Identifier: NCT04331808

Résumé : Radiometric measurements in the Thermal Infrared (TIR) domain exhibit an angular variation over most surface types, known as the Thermal Radiation Directionality (TRD) phenomenon. A primary objective of the ongoing development of TRD physical models is to perform a correction of the angular effects to obtain comparable land surface temperature products. In practice, it is advised to handle only the models having a limited number of input parameters for the purpose of operational applications. The use of semi-empirical kernel-driven models (KDMs) appears to be a good tradeoff between physical accuracy and computational efficiency as it was already demonstrated through a broad usage in the optical domain. It remains that the existing state-of-the-art 3-parameter TIR KDMs (RossThick-LiSparseR, LiStrahlerFriedl-LiDenseR, Vinnikov, and RoujeanLagouarde) underestimate the hotspot phenomenon, especially for continuous canopies marked by a narrow peak. In this study, a new general framework of TIR kernel-driven modeling is proposed to overcome such issue. It is a linear combination of three kernels (including a base shape kernel, a hotspot kernel with adjustable width and an isotropic kernel) with the ability to simulate the bowl, dome and bell shapes in the solar principal plane. Four specific 4-parameter models (Vinnikov-RoujeanLagouarde, LiStrahlerFriedl-RoujeanLagouarde, Vinnikov-Chen, and LiStrahlerFriedl-Chen, named “base shape kernel - hotspot kernel”) within the new framework were studied to assess their abilities to mimic the patterns of the directional brightness temperature for both continuous and discrete vegetation canopies. These four 4-parameter KDMs and four 3-parameter KDMs were comprehensively evaluated with 306 groups of simulated multi-angle datasets generated by a modernized analytical 4-stream radiative transfer model based on the Scattering by Arbitrarily Inclined Leaves (4SAIL), and a Discrete Anisotropic Radiative Transfer (DART) model considering different solar zenith angles (SZA), canopy architectures and component temperatures, and 2 groups of airborne measured multi-angle datasets over continuous maize and discrete pine forest. Results show that the four 4-parameter KDMs behave better than the four existing 3-parameter KDMs over continuous canopies (e.g. R2 increases from 0.661~0.970 to 0.940~0.997 and RMSE decreases from 0.17~0.71 to 0.07~0.16 when SZA = 30°) and discrete canopies (e.g. R2 increases from 0.791~0.989 to 0.976~0.996 and RMSE decreases from 0.10~0.84 to 0.08~0.21 when SZA = 30°). The new general framework with four parameters (three kernel coefficients and an adjustable hotspot width) improves the fitting ability significantly, compared to the four existing three-parameter KDMs, given the addition of one more degree of freedom. Results show that the coefficients of the base shape kernel, hotspot kernel and isotropic kernel are related to the temperature difference between leaf and background, temperature difference between sunlit component and shaded component, and the nadir brightness temperature, respectively. However, the estimated hotspot width depends on vegetation structure. The new kernel-driven modeling framework has the potential to be a tool for angular correction of multi-angle satellite observations and angular optimization of future multi-angle TIR sensors.

Résumé : RationaleHypermetabolism (HM) in Amyotrophic lateral sclerosis (ALS) is the reflection of a high energy metabolic level, but this alteration seems controversial. The main objective of the study was to confirm the existence of HM during ALS compared to healthy subjects.MethodsA cohort of ALS patients was compared to a control group without metabolic disorder. The assessment included anthropometric criteria measurements, body composition by bioelectric impedance analysis and resting energy expenditure (REE) by indirect calorimetry. HM was defined as a variation > +10% between measured and calculated REE. Statistical analysis used Mann-Withney and Chi2 tests. Multivariate analysis included logistic regression.Results287 patients and 75 controls were included. The metabolic level was higher in ALS patients (1500 kcal/24 h [1290–1693] vs. 1230 kcal/24 h [1000–1455], p < 0.0001) as well as the REE/fat free mass ratio (33.5 kcal/kg/24 h [30.4–37.8] vs. 28.3 kcal/kg/24 h [26.1–33.6], p < 0.0001). 55.0% of ALS patients had HM vs. 13.3% of controls (p < 0.0001). HM was strongly and positively associated with ALS (OR = 9.50 [4.49–20.10], p < 0.0001).ConclusionsHM in ALS is a reality, which affects more than half of the patients and is associated with ALS. This work confirms a very frequent metabolic deterioration during ALS. The identification of HM can allow a better adaptation of the patients' nutritional intake.

Résumé : Background: There is no evidence to support surgery or radiotherapy as the best treatment for resectable oropharyngeal cancers with a negative HPV status. Predictive algorithms may help to decide which strategy to choose, but they will only be accepted by caregivers and European authorities if they are interpretable. As a proof of concept, we developed a predictive and interpretable algorithm to predict locoregional relapse at 18 months for oropharyngeal cancers as a first step towards that goal. Methods: The model was based on clinical and Pyradiomics features extracted from the dosimetric CT scan. Intraclass correlation was used to filter out features dependant on delineation. Correlated redundant features were also removed. An XGBoost model was cross-validated and optimised on the HN1 cohort (79 patients), and performances were assessed on the ART ORL cohort (45 patients). The Shapley Values were used to provide an overall and local explanation of the model. Results: On the ART ORL cohort, the model trained on HN1 yielded a precision—or predictive positive value—of 0.92, a recall of 0.42, an area under the curve of the receiver operating characteristic of 0.68 and an accuracy of 0.64. The most contributory features were shape Voxel Volume, grey level size zone matrix Small Area Emphasis (glszmSAE), gldm Dependence Non Uniformity Normalized (gldmDNUN), Sex and Age. Conclusions: We developed an interpretable and generalizable model that could yield a good precision—positive predictive value—for relapse at 18 months on a different test cohort.

Résumé : NGC 4330 is one of the Virgo galaxies whose UV emission distributions show a tail structure. An associated tail structure is also observed in the HI and Hα emission distributions. Previous dynamical modeling has shown that the galaxy is approaching the cluster center and is therefore undergoing increasing ram pressure stripping. Recent stellar population fitting of deep optical spectra together with multiband photometry led to the determination of the time when star formation was quenched in the galactic disk. We introduce a new version of the dynamical model that includes not only the dense neutral gas, but also the diffuse ionized gas, and we aim to reproduce the HI, Hα, and UV distributions together with the star formation histories of the outer gas-free parts of the galactic disk. We present the results of 50 simulations with five different Lorentzian temporal ram pressure profiles and five different delays between the simulation onset and peak ram pressure. The delays were introduced to study the influence of galactic structure on the outcome of the simulations. The inclusion of diffuse gas stripping significantly changes the HI, UV, and Hα emission distributions. The simulations with diffuse gas stripping naturally led to vertical low surface density filaments in the downwind region of the galactic disk. These filaments occur less frequently in the simulations without diffuse gas stripping. The simulations with diffuse gas stripping led to better joint fits to the spectral energy distributions (SEDs) and optical spectra. The HI, near-UV, and Hα morphologies of the model snapshots that best reproduce the SEDs and optical spectra are sufficiently different to permit a selection of best-fit models. We conclude that the inclusion of diffuse gas stripping significantly improves the resemb+lance between the model and observations. Our preferred model yields a time to peak ram pressure of 140 Myr in the future. The spatial coincidence of the radio continuum and diffuse Hα tails suggests that both gas phases are stripped together. We suggest that the star formation in the outer tail is sporadic and low level, and this explains the absence of a significant amount of cosmic ray electrons there. Furthermore, we suggest that the mixed interstellar medium is ionized by collisions with the thermal electrons of the ambient intracluster medium, which confines the filaments.
