Résumé : VEXAS (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) syndrome is an autoinflammatory disorder caused by acquired somatic UBA1 mutations in hematopoietic stem cells, affecting peripheral myeloid and natural killer (NK) cells. Given the high rate of severe infections observed in VEXAS patients, we hypothesized that NK cell dysfunction contributes to this increased susceptibility. We conducted a comprehensive immune characterization of peripheral NK cells in patients with VEXAS (n=40), patients with autoinflammatory diseases without UBA1 mutations (n=22), and elderly gender-matched healthy controls (HCs) (n=16). Multiparameter phenotyping was performed using CyTOF, scRNAseq, whole blood stimulation assays and in vitro NK cell cytotoxic assay. Peripheral NK cells in VEXAS were quantitatively and qualitatively impaired. Mass cytometry revealed reduced frequencies of mature cytotoxic CD56dim NK cells and an expansion of the CD56high CD16dim subset. NK cells exhibited features of exhaustion, including increased PD-1 expression, and reduced levels of cytotoxic markers such as NKp46 and CD8α. scRNAseq analysis showed decreased signatures of cytotoxicity and IL-2 and IFN-γ production, alongside increased inflammatory signatures. Whole blood stimulation assays confirmed impaired IL-2, IFN-γ, and granzyme B production following TLR3, TLR4, and TLR7/TLR8 agonist stimulation. Extended NK phenotyping by flow cytometry confirmed reduced activating receptors' expression and impaired IFN-γ production in VEXAS. Moreover, in vitro UBA1 inhibitors impaired NK cell cytotoxic capacity and promote cell death. Finally, reduced NK cell frequencies were independently associated with an increased risk of severe infections. These findings suggest that NK cell dysfunction in VEXAS syndrome contributes to increased susceptibility to severe infections.

Résumé : The 2025 update of the French guidelines for localized renal cell carcinoma highlights several major advances. Renal biopsy reaffirms its central role in guiding therapeutic decisions and reducing overtreatment, with growing evidence supporting a broader "biopsy-all" strategy. Active surveillance is now established as a safe and effective option for small renal masses, including selected complex cystic lesions, providing oncological outcomes comparable to immediate intervention. Among alternative approaches, stereotactic body radiotherapy has emerged as a credible and well-tolerated treatment for medically inoperable patients. In hereditary syndromes, progress includes improved characterization of BAP1-TPDS-associated RCC and the expanding role of belzutifan in von Hippel-Lindau disease. From an organizational standpoint, the center-volume effect is confirmed, supporting the centralization of renal cancer surgery within authorized, high-volume centers. Novel biomarkers such as KIM-1, urinary glycosaminoglycans, and circulating DNA represent promising tools for postoperative surveillance and personalized treatment planning. In the event of metastatic progression after adjuvant pembrolizumab, enrolment in a clinical trial remains the preferred strategy. For early relapse (during or within six months after adjuvant therapy), TKI monotherapy should be prioritized, whereas recurrence between six and twelve months may justify the reintroduction of immunotherapy. Beyond twelve months after pembrolizumab, standard first-line PD-1-based combinations remain the recommended approach. Finally, for localized or oligometastatic recurrences, local treatments should be considered within a multidisciplinary tumour board.

Résumé : Context . The Near InfraRed Planet Searcher (NIRPS) joined HARPS on the 3.6-m ESO telescope at La Silla Observatory in April 2023, dedicating part of its Guaranteed Time Observations (GTO) program to the radial velocity follow-up of TESS planet candidates to confirm and characterize transiting planets around M dwarfs. Aims . We present the “Sub-Neptunes” subprogram of the NIRPS-GTO, aimed at investigating the composition and formation of sub-Neptunes orbiting M dwarfs. We report the first results of this program with the characterization of the TOI-756 system, which consists of TOI-756 b, a transiting sub-Neptune candidate detected by TESS, as well as TOI-756 c, an additional non-transiting planet discovered by NIRPS and HARPS. Methods . We analyzed TESS and ground-based photometry, high-resolution imaging, and high-precision radial velocities (RVs) from NIRPS and HARPS to characterize the two newly discovered planets orbiting TOI-756, as well as to derive the fundamental properties of the host star. A dedicated approach was employed for the NIRPS RV extraction to mitigate telluric contamination, particularly when the star’s systemic velocity was shown to overlap with the barycentric Earth radial velocity. Results . TOI-756 is a M1V-type star with an effective temperature of T eff ~ 3657 K and a super-solar metallicity ([Fe/H]) of 0.20±0.03 dex. TOI-756 b is a 1.24-day period sub-Neptune with a radius of 2.81 ± 0.10 R ⊕ and a mass of 9.8 −1.6 +1.8 M ⊕ . TOI-756 c is a cold eccentric (e c = 0.45 ± 0.01) giant planet orbiting with a period of 149.6 days around its star with a minimum mass of 4.05 ± 0.11 M Jup . Additionally, a linear trend of 146 m s −1 yr −1 is visible in the radial velocities, hinting at a third component, possibly in the planetary or brown dwarf regime. Conclusions . We present the discovery and characterization of the transiting sub-Neptune TOI-756 b and the non-transiting eccentric cold giant TOI-756 c. This system is unique in the exoplanet landscape, standing as the first confirmed example of such a planetary architecture around an M dwarf. With a density of 2.42 ± 0.49 g cm −3 , the inner planet, TOI-756 b, is a volatile-rich sub-Neptune. Assuming a pure H/He envelope, we inferred an atmospheric mass fraction of 0.023 and a core mass fraction of 0.27, which is well constrained by stellar refractory abundances derived from NIRPS spectra. It falls within the still poorly explored radius cliff and at the lower boundary of the Neptune desert, making it a prime target for a future atmospheric characterization with JWST to improve our understanding of this population.

Résumé : Souvent considéré comme une simple formalité marquant la fin du traitement, le débaguage orthodontique est en réalité une étape à haut risque iatrogène. Réalisé parfois de manière « précipitée », il peut entraîner des dommages irréversibles sur un émail fragilisé. Un paradoxe, compte tenu de la patience imposée durant des mois, voire des années de traitement. Bien que des données existent dans la littérature, le manque d’informations, de formation ou la sous-estimation des dégâts potentiels conduisent à sacrifier, parfois involontairement, la qualité au profit de la rapidité. Cet article propose de replacer le débaguage au centre des préoccupations cliniques. Il s’intéresse aux limites des instruments actuellement disponibles, tout en ouvrant des pistes vers une instrumentation plus précise, mieux tolérée par les tissus dentaires et compatible avec les contraintes de temps de la pratique clinique quotidienne.

Résumé : Background: Clinical trials are designed to prove the efficacy of an intervention by means of model-based approaches involving parametric hypothesis testing. Issues arise when no effect is observed in the study population. Indeed, an effect may be present in a subgroup and the statistical test cannot detect it. To investigate this possibility, we proposed to change the paradigm to a data-driven approach. We selected exploratory methods to provide another perspective on the data and to identify particular homogeneous subgroups of subjects within which an effect might be detected. In the setting of prevention trials, the endpoint is a trajectory of repeated measures. In the settings of prevention trials, the endpoint is a trajectory of repeated measures, which requires the use of methods that can take data autocorrelation into account. The primary aim of this work was to explore the applicability of different methods for clustering and classifying trajectories. Methods: The Multidomain Alzheimer Preventive Trial (MAPT) was a three-year randomized controlled trial with four parallel arms (NCT00672685). The primary outcome was a composite Z-score combining four cognitive tests. The data were analyzed by quadratic mixed effects model. This study was inconclusive. Exploratory analysis is therefore relevant to investigate the use of data-driven methods for trajectory classification. The methods used were unsupervised: k-means for longitudinal data, Hierarchical Cluster Analysis (HCA), graphic semiology, and supervised analysis with dichotomous classification according to responder status. Results: Using k-means for longitudinal data, three groups were obtained and one of these groups showed cognitive decline over the three years of follow-up. This method could be applied directly to the primary outcome, the composite Z-score with repeated observations over time. With the two others unsupervised methods, we were unable to process longitudinal data directly. It was therefore necessary to choose an indicator of change in trajectories and to consider the rate of change between two measurements. For the HCA method, Ward's aggregation was performed. The Euclidean distance and rates of change were applied for the graphic semiology method. Lastly, as there were no objective criteria to define responder status, we defined our responders based on clinical criteria. Discussion: In the princeps study, the prevention trial was found to be inconclusive, likely due to the heterogeneity of the population, which may have masked a treatment effect later identified in a refined subgroup of high Beta Amyloid subjects. So, we have adopted an alternative unsupervised approach to subject stratification based on their trajectories. We could then identify patterns of similar trajectories of cognitive decline and also highlight the potential problem of a large heterogeneity of the profiles, maybe due to the final endpoint considered.

Résumé : Diamonds and nanodiamonds (NDs) containing quantum color centers, such as nitrogen-vacancy (NV) centers, have been widely studied in recent years for their promising applications across various fields, including cryptography and telecommunications, medical sciences, and sensing. In the context of sensing, recent studies on color centers such as silicon-vacancy (SiV) and germanium-vacancy (GeV) centers have shown great potential for measuring physical quantities like magnetic fields, temperature, and strain. This is due to their intense zero-phonon line (ZPL) optical signal, about 80% of their total luminescence, which is characteristic of group-IV color centers (G4V) [1]. In our recent papers, we demonstrated that as-grown CVD NDs containing SiV and GeV centers, featuring luminescent ZPLs and excellent photostability at room temperature, can function effectively as nanosensors of high stress and pressure conditions (up to 180 GPa) [2, 3]. To advance our research, in this work we investigate the optical properties of SiV centers in as-grown NDs at cryogenic temperatures. For SiV centers, which have spin 1/2, the fine structure should become accessible through the Jahn-Teller effect, which lifts the degeneracy at cryogenic temperatures (< 20 K). However, experiments on our as-grown NDs reveal significant spectral broadening even at around 9 K, caused by lattice strain, which prevents access to the optically addressable electronic spin states of the SiV center. To overcome this limitation, we propose an original post-treatment involving high-pressure and high-temperature (HPHT) annealing using the Paris–Edinburgh press. In fact, the gradual increase of temperature and pressure allows for control over the undesired diamond-to-graphite phase transition. HPHT annealing experiments were conducted at the PSICHE beamline of Synchrotron SOLEIL [4], where X-ray diffraction and tomography analyses enabled precise monitoring of P,T parameters and made it possible for the first time to access single fine-structure transitions of the in such SiV-NDs. Résumé : Enfin le grand jour ! Celui attendu avec impatience, parfois avec un brin d’appréhension : LA dépose de l’appareil orthodontique. Ce moment symbolique où le patient, prêt à chanter « Libéré, délivré », sent souffler un vent de liberté. Un parcours de soins partagé s’achève, souvent long, ponctué de réévaluations et parfois de plusieurs appareils… Mais c’est aussi une tranche de vie vécue aux côtés des patients et de leurs familles, avec des liens qui se sont tissés au fil du temps. Et pour nous, Orthodontistes, ce moment va bien au-delà du simple retrait de l’appareil : il marque le passage du traitement actif à la phase de contention. Un vaste sujet, la contention… Mais aujourd’hui, concentrons-nous sur l’étape intermédiaire : la fameuse dépose de la colle orthodontique restante !

Résumé : In multicellular organisms, cell division shapes tissue architecture, cell identity, and function. In walled organisms like plants, division plane orientation irreversibly defines tissue topology and is tightly regulated. While divisions often follow the shortest path enclosing cell volume, certain cells deviate, dividing perpendicular to the growth axis. Tissue-scale mechanical stress has been proposed to guide such orientation, but how these cues are integrated remains unclear. Here, we re-examine the role of the actin cytoskeleton in orienting cell division in Arabidopsis root epidermis. Combining cell biology, genetics, pharmacological treatments, 3D segmentation, and modelling, we show that actin is a central molecular actor required to establish cell division orientation against the geometrical rules, highlighting its role in integrating spatial information.

Résumé : Aims: There is a lack of studies investigating the pathophysiologic and phenotypic distinctiveness of aortic stenosis (AS). This heterogeneity has important implications for identifying optimal intervention timing and potential medical management. This study seeks to identify phenogroups of AS using unsupervised machine learning to improve risk stratification. Methods and results: A total of 349 patients with asymptomatic AS from the PROGRESSA study were included in this analysis. Echocardiographic, clinical and blood sample data were used in the unsupervised clustering process. Longitudinal echocardiographic data were used to evaluate AS progression. Five clusters of patients were revealed using 18 variables selected by an unsupervised machine learning algorithm. Amongst them, aortic valvular phenotype, mean gradient, peak jet velocity (Vpeak), and left ventricle stroke volume were selected as discriminatory variables. Following the clustering process, characteristics differed between clusters, including age, body mass index, and sex ratio (all P < 0.001). Of note, cluster 1 showed higher AS severity at baseline with significantly higher initial Vpeak (344 [314; 376] cm/s) and calcium score (1257 [806; 1837] UA) (P < 0.001). Patients from cluster 1 had a faster AS progression (progression of Vpeak = 22 [9; 39] cm/s/year), and calcium score (213 [111; 307] UA/year) (P < 0.001). Cluster 1 was also associated with a higher composite risk of mortality and aortic valve replacement when adjusted for age, sex, and baseline AS severity (P < 0.001). Conclusion: Artificial intelligence-guided phenotypic classification revealed 5 distinct groups and enhanced risk stratification of patients with AS. This approach may be useful to optimize and individualize medical and interventional management of AS.
