Résumé : Objective. To develop a multidisciplinary French framework addressing neurosurgical management in the initial phase of traumatic brain injury (TBI) in adults and children.Design. A panel of 29 experts was formed at the request of the French Society of Neurosurgery (SFNC), with the participation of the French Society of Pediatric Neurosurgery (SFNCP), French Society of Private-Practice Neurosurgeons (SFNCL), French-Speaking Neurocritical Care and Neuro-Anesthesiology Society (ANARLF), French Society of Anesthesia, Critical Care and Perioperative Medicine (SFAR), French-Speaking Pediatric Emergency and Intensive Care Group (GFRUP), French Society of Neuroradiology (SFNR), French-Speaking Infectious Diseases Society (SPILF), and the French Society of Physical Medicine and Rehabilitation (SOFMER).Methods. Questions were formulated using the PICO (Patients, Intervention, Comparison, Outcome) format, grouped into 7 categories: 1. Factors of poor prognosis, 2. Extradural hematoma, 3. Acute subdural hematoma, 4. Skull-base fracture and dural tear, 5. Penetrating traumatic brain injury, 6. Post-traumatic cerebrospinal fluid disorder, and 7. Pediatric specificities.Results. Synthesis by the experts and application of the GRADE® method resulted in the formulation of 45 recommendations. Strong consensus was reached for all recommendations at the first round of rating,Conclusion. There was a strong consensus among the experts on important interdisciplinary recommendations to improve the neurosurgical management of patients with TBI.

Résumé : Background: Real-world data suggest that the early use of highly active therapies (HAT) may reduce the risk of transition to secondary progressive MS (SPMS). However, current knowledge about predictive factors of outcomes needs to be improved. The primary objective of this study was to determine factors associated with the occurrence of SPMS in patients treated early after MS onset with an HAT. Methods: Retrospective, multicentric study based on the French MS database. Patients who initiated a HAT within 5 years after MS onset, EDSS ⩽4, and had a follow-up >5 years were included. The association of each covariate at baseline with time to the occurrence of SPMS was quantified by hazard ratios (HRs) in unadjusted and adjusted Cox proportional hazards models. Results: Two thousand two hundred and thirty-seven patients were included in the analysis: mean age 31.6 years, female/male sex ratio 2.3, and median EDSS 2.0. The estimated probability of reaching SPMS, progression independent of relapse activity (PIRA) and progression independent of activity (PIA) at 10 years was 8%, 22%, and 11%, respectively. After adjustment, we found that female patients (HR 0.64, p = 0.036) had a lower risk of developing SPMS. Older age, EDSS >0 (HR 7.44, p < 0.001), and oral versus intravenous HAT (HR 1.97, p = 0.003) were significantly associated with an increased SPMS risk. Early PIRA and PIA predicted conversion to SPMS. Conclusions: Early HAT use resulted in a low risk of developing SPMS over 10 years. Introducing the HAT before any residual disability was associated with a lower risk of progression.

Résumé : It is essential to assess the impact of future climate change on catchment hydrology in a rigorous manner, accounting for uncertainties. In this study, we assess the impact of climate change on the natural streamflow of the French part of the Meuse catchment. Climate projections from two Representative Concentration Pathways (RCPs) and five General Circulation Model/Regional Climate Model (GCM/RCM) couples were retrieved to feed four hydrological models run with several parameter sets to assess future streamflow. A variance analysis tool was employed to partition the sources of uncertainty. Although an increase in air temperature is expected in the future, particularly with the RCP 8.5, the projected trend in precipitation remains uncertain. A slight increase in annual precipitation, an increase in winter precipitation and an uncertain signal for summer precipitation are anticipated. These developments will result in an increase in winter streamflow and an uncertain signal for summer streamflow, although these evolutions may not be homogeneous along the Meuse course. Regarding uncertainties, internal variability represents the greatest uncertainty in the near future, while climate models account for the highest uncertainty in the mid and far futures. The results were found to be comparable to those of the Francewide Explore2 project.

Résumé : In the context of space missions, an efficient data transfer strategy is crucial to avoid onboard buffer overflow. In many missions, such as the ESA's Rosetta mission, the data produced by each instrument is temporarily stored in a dedicated buffer and then dumped to Earth during downlink windows under a limited bandwidth. Previous work has considered the overlapping Memory Dumping Problem (oMDP), which consists in assigning transfer priorities to the memory buffers and minimize the peaks memory usage. The oMDP is NP-hard and has been tackled using heuristic methods that demonstrated their efficiency on real instances. In this paper, we consider additional decisions in the memory dumping plans that are implementable in practice: data transfer from each buffer can be interrupted after a given time, once per downlink window, preventing it from dumping data until the next window. The new problem is called oMDPi (oMDP with interruptions). We obtain several structural properties and complexity results, showing in particular that oMDPi is NP-hard for at least two windows. While the complexity status of the single window oMDPi remains open, we propose a polynomial-time heuristic to solve it. We propose a hybrid heuristic to solve the general oMDPi, embedding a flow relaxation and the single window heuristic. The results on both real and realistic generated instances show that our heuristic achieves a significant reduction of memory peaks in a reasonable time compared to previous works, making the new policy attractive for future space missions.

Résumé : Water vapor information in the upper troposphere (UT) is crucial for understanding the thermodynamic conditions leading to the formation of cirrus clouds and persistent contrails. Both phenomena significantly contribute to aviation-induced radiative forcing, driving global mitigation efforts. Raman lidars provide high-resolution humidity profiles, describing altitudes prone to ice supersaturation—conditions that are challenging to detect and accurately represent in current models.In this study, Raman lidar Water Vapor Mixing Ratio (WVMR) measurements from various sites in France were used to evaluate the performance of the ERA5 model in assessing humidity at typical aircraft altitudes. Additionally, the uncertainties in Microwave Limb Sounder (MLS) WVMR measurements at the same altitudes were assessed. Raman lidar profiles were aggregated into pseudo-monthly datasets to facilitate comparison with the limited number of MLS overpasses at each site, enabling validation of spatio-temporal pseudo-monthly lidar-matched MLS and ERA5 WVMR profiles.The MLS dataset offers one of the longest records of WVMR, making it a valuable resource for trend assessment. This investigation enables the validated use of these datasets for studying UT humidity trends and variability on seasonal and annual scales over the past decade.

Résumé : According to the Global Burden of Diseases, PM2.5 (particles with a diameter under 2.5 µm) is the leading cause of diseases and deaths in 2021 (Brauer et al., 2024). Along with decreased emissions, removal through deposition is used to reduce exposure to particulate matter (PM). With a leaf area index (m² of leaf per m² of land) usually higher than 1, plants allow for a higher deposition surface, hence more particle removal from ambient air. Thus, understanding and estimating PM deposition on vegetation is necessary to assess the impact of vegetation on air quality. In situ measurements above vegetation have shown that PM (vertical) deposition velocity can be positive and negative (Pellerin et al., 2017). No 1-dimensional PM deposition model can predict such values. The objective of this study is to implement a working bi-directional PM exchange scheme in the Surfatm exchange model (Personne et al., 2024), a 1-dimensional SVAT model, using a resistive scheme. The bi-directional fluxes are introduced using a compensation point approach, which can be interpreted as the PM surface concentration. This allows the concentration gradient to change signs depending on the difference of concentration between ambient air and the surface. Two PM exchange datasets above a grassland in Lusignan (France) are used to calibrate and validate the model respectively (Pellerin et al., 2017). The Surfatm-PM model can predict positive and negative deposition velocities, with notable differences attributable to the formation mechanism of the particles, such as the process of coagulation or nucleation or condensation between ambient air and (vegetated) interfaces.

Résumé : The purpose of this study was to assess the prognostic value of 18F-FDG PET parameter variation between baseline and 42 Gy (PET2) of radiochemotherapy at 6 mo and 1 y of evaluation in patients with stage III inoperable nonsmall cell lung cancer based on RECIST 1.1. Methods: In total, 158 patients in a prospective multicenter phase II/III study were analyzed. Patients were randomized into 2 groups: an experimental arm (group A) and a standard arm (group B). Patients from group A with residual metabolism on PET2 (group A+) at 42 Gy received a radiation boost (74 Gy). Patients without residual uptake on 18F-FDG PET at 42 Gy (group A-) and patients in group B received a standard radiotherapy dose (66 Gy). We compared group A with group B. The 18F-FDG PET parameters SUVmax, SUVmean, SUVpeak, peak SUV normalized on lean body mass, mean SUV normalized on lean body mass, total lesion glycolysis, total metabolic tumor volume (MTV) (tumor and nodes), and tumor MTV were measured. All patients were evaluated with RECIST 1.1 using CT at 6 mo and 1 y after radiochemotherapy. Progression-free survival and overall survival were evaluated. Results: Except for the radiotherapy dose (P < 0.001), patient demographic characteristics were similar between the 2 groups (A vs. B). All 18F-FDG PET uptake and volume parameter measurements were correlated. Therefore, only the change in SUVmax (ΔSUVmax) and total MTV were selected for the analysis. There was no significant difference in any variable between the 2 groups. In the multivariate analysis, ΔSUVmax appeared to be the most important prognostic factor for overall survival, and SUVmax of PET2 appeared to be the most important prognostic factor for progression-free survival. Conclusion: 18F-FDG PET at 42 Gy can be used to identify good responders to radiochemotherapy in patients with inoperable stage III nonsmall cell lung cancer. The SUVmax of PET2 and ΔSUVmax are independent prognostic factors.

Résumé : We investigated whether baseline levels of biomarkers related to endotheliopathy, thromboinflammation, and fibrosis were associated with clinical outcomes in hospitalized COVID-19 patients. We analyzed the associations between baseline levels of 21 biomarkers and time to hospital discharge and change in NEWS-2 score in patients from DisCoVeRy trial. We fitted multivariate models adjusted for baseline ISARIC 4C score, disease severity, D-dimer values, and treatment regimen. Between March 22 and June 29, 2020, 603 participants were randomized; 454 had a sample collected at baseline and analyzed. The backward selection of multivariate models showed that higher baseline levels of soluble suppressor of tumorigenicity 2 (sST2) and nucleosomes were statistically associated with a lower chance of hospital discharge before day 29 (sST2: aHR 0.24, 95% CI [0.15-0.38], p < 10-9; nucleosomes: aHR 0.62, 95% CI [0.48-0.81], p < 10-3). Likewise, higher levels of baseline sST2 were statistically associated with lower changes in the NEWS-2 score between baseline and day 15 (adjusted beta 4.47, 95% CI [2.65-6.28], p < 10-5). Moreover, we evaluated sST2 involvement in a confirmation cohort (SARCODO study, 103 patients) and found that elevated baseline sST2 levels were significantly associated with lower rates of hospital discharge before day 29 and a higher model performance (AUC at day 29 of 92%) compared to models without sST2. sST2 emerged as an independent predictor of clinical outcomes in two large cohort of hospitalized COVID-19 patients, warranting further investigation to elucidate its role in disease progression and potential as a therapeutic target.
