Résumé : INTRODUCTION: The incidence of genitourinary tumors (GUT) in renal transplant recipients (RTR) is higher than in the general population. We previously reported that CD4 lymphocytopenia is associated with a high incidence of skin cancer in RTR. Here, we investigate whether persistent CD4 T cell lymphopenia is associated with GUT occurrence. PATIENTS AND METHODS: A total of 433 patients were included in this study. All patients underwent annually systematic lymphocyte subset (CD3, CD4, CD8, CD19) determination by flow cytometry. RESULTS AND CONCLUSION: During the follow-up period, 13 patients developed GUT: 6 patients a prostate adenocarcinoma (incidence 0.06%/year) and 7 patients a renal cell carcinoma (incidence 0.07%/year). The patients with GUT were older than those without. Both groups did not differ in posttransplant duration, dialysis mode and duration, induction regimen, or acute rejection history. No persistent CD4 lymphopenia was observed in the patients with GUT. Although CD4 T cell lymphopenia is associated with skin cancer in long-term RTR, it did not appear to be a risk factor for GUT. This suggests that other factors encountered in the setting of kidney transplantation (e.g., immunosuppressive drugs, end-stage renal failure, etc.) favor the development of GUT in RTR.

Résumé : After the demonstration that no Châtelperronian/Aurignacian interstratifications existed at Roc-de-Combe, Le Piage, and El Pendo, the Grotte des Fées remained the single instance conceivably documenting the putative longterm regional contemporaneity of the two technocomplexes which is required by the “acculturation” and “imitation” models of the innovations (ornaments, decorated bone tools) that characterize the Châtelperronian. Based on our own inspection and study of the lithic and faunal collections and of the site itself, on consideration of the published and unpublished literature, and on the available dating evidence, we show that the putative interstratified deposits reported by Henri Delporte, who last excavated at the Grotte des Fées in 1951–54 and 1962, are in fact 19th-century backfill. In situ remnants may have been found in 1962, but, in the part of the site excavated at that time, Delporte failed to recognize the previously described interstratification. The stratigraphic consistency of 10 (out of the 13) radiocarbon dates available relates to the fact that those 10 dates are on samples from the conceivably in situ remnants excavated in 1962. Three samples from Level B5 date the Châtelperronian occupation to ca 39–40,000 14C BP, while seven samples from Level B1–3 relate to carnivore denning at the site throughout the ca 36,500–34,500 14C BP interval. Subsequent sporadic incursions by bearers of an Aurignacian I tool-kit are documented by diagnostic material scattered from top to bottom of the backfill deposits excavated by Delporte between 1951 and 1954. Originally, that Aurignacian material must have been contained in a surficial occupation lens capping the site's Mousterian-to-Châtelperronian stratigraphic succession.

Résumé : BACKGROUND: Indolent forms of mastocytosis account for more than 90% of all cases, but the types and type and severity of symptoms and their impact on the quality of life have not been well studied. We therefore performed a case-control cohort study to examine self-reported disability and impact of symptoms on the quality of life in patients with mastocytosis. METHODOLOGY/PRINCIPAL FINDINGS: In 2004, 363 mastocytosis patients and 90 controls in France were asked to rate to their overall disability (OPA score) and the severity of 38 individual symptoms. The latter was used to calculate a composite score (AFIRMM score). Of the 363 respondents, 262 were part of an ongoing pathophysiological study so that the following data were available: World Health Organization classification, standard measures of physical and psychological disability, existence of the D816V KIT mutation, and serum tryptase level. The mean OPA and AFIRMM scores and the standard measures of disability indicated that most mastocytosis patients suffer from disabilities due to the disease. Surprisingly, the patient's measurable and perceived disabilities did not differ according to disease classification or presence or absence of the D816V KIT mutation or an elevated (> or = 20 ng/mL) serum tryptase level. Also, 32 of the 38 AFIRMM symptoms were more common in patients than controls, but there were not substantial differences according to disease classification, presence of the D816V mutation, or the serum tryptase level. CONCLUSIONS: On the basis of these results and for the purposes of treatment, we propose that mastocytosis be first classified as aggressive or indolent and that indolent mastocytosis then be categorized according to the severity of patients' perceived symptoms and their impact on the quality of life. In addition, it appears that mastocytosis patients suffer from more symptoms and greater disability than previously thought, that mastocytosis may therefore be under-diagnosed, and that the symptoms of the indolent forms of mastocytosis might be due more to systemic release of mediators than mast cell burden.

Résumé : We present a theoretical model for the propagation of non-self-interacting atom laser beams. We start from a general propagation integral equation and we use the same approximations as in photon optics to derive tools to calculate the atom-laser-beam propagation. We discuss the approximations that allow one to reduce the general equation whether to a Fresnel-Kirchhoff integral calculated by using the stationary phase method, or to the eikonal. Within the paraxial approximation, we also introduce the ABCD matrices formalism and the beam quality factor. As an example, we apply these tools to analyze the recent experiment.

Résumé : Several studies suggest that (n-3) PUFA may play a role in the regulation of cognitive functions, locomotor and exploratory activity, and affective disorders. Additionally, (n-3) PUFA affect pineal function, which is implicated in the sleep-wake rhythm. However, no studies to our knowledge have explored the role of PUFA on the circadian system. We investigated the effect of an (n-3) PUFA-deficient diet on locomotor and pineal melatonin rhythms in Syrian hamsters used as model species in circadian rhythm research. To assess the possible relationship between voluntary wheel running activity and dopaminergic neurotransmission, we also measured endogenous monoamine concentrations in the striatum. Two-month-old male hamsters, fed either an (n-3) PUFA-deficient or an (n-3) PUFA-adequate diet, were housed individually in cages equipped with run wheels. At 3 mo, cerebral structures were extracted for biochemical and cellular analysis. In (n-3) PUFA-deficient hamsters, the induced changes in the pineal PUFA membrane phospholipid composition were associated with a reduction in the nocturnal peak level of melatonin that was 52% lower than in control hamsters (P < 0.001). The (n-3) PUFA-deficient hamsters also had higher diurnal (P < 0.01) and nocturnal (P = 0.001) locomotor activity than the control hamsters, in parallel with activation of striatal dopaminergic function (P < 0.05). The (n-3) PUFA-deficient hamsters exhibited several symptoms: chronic locomotor hyperactivity, disturbance in melatonin rhythm, and striatal hyperdopaminergia. We suggest that an (n-3) PUFA-deficient diet lessens the melatonin rhythm, weakens endogenous functioning of the circadian clock, and plays a role in nocturnal sleep disturbances as described in attention deficit/hyperactivity disorder.
